Data Quality Assistant - General Practice

Job summary

An opportunity has arisen for a Data Quality Assistant to work within our friendly Practice Team.

If you have experience of working in General Practice, have attention to detail, and always put the patient first, we would welcome your application.

Main duties of the job

  • To process incoming electronic and paper test results, clinic letters, A&E and 111 reports etc.
  • Scanning to the patient record.
  • Reading each document thoroughly.
  • Adding clinical codes to the medical record.
  • Forwarding document to the relevant clinician for onward action.
  • You will work to a high standard to ensure that patient records are maintained accurately and that correct codes are used at all times.
  • Following Practice Policy on receipt of Child Protection documentation.
  • Actioning deductions (where patients have died, or left the practice, notes to be pulled; electronic labels to be requested. Notes to be packaged securely and put ready for the PCSE Courier.
  • Having an awareness of the GSF patients; and ensuring they are coded appropriately, and receive gold standard care.
  • Action of daily tasks and patient notes.
  • Assist the Data Quality Lead on projects that may arise.
  • Liaise via telephone with patients, outside agencies and other Health Professionals
  • Inform (by task) the Cancer Care Champion of any new cancer diagnoses.
  • To develop a knowledge of searches through basic training; assist with audits and data checking.
  • To learn basic maintenance functions of EMIS web to help with the effective workings of the practice.
  • Assist in Clinical Governance, Information Governance and CQC programmes where appropriate.
  • To undertake other duties as required.

Accountable to: Practice Manager

Base: Sandy Lane Surgery

Nature of the Role

To support the Data Quality function for the practice to enhance care for patients, to include Data Quality and Workflow.

Job Responsibilities

  • To process the daily workflow: (Docman, daily correspondence, EDTs [electronic communication from Secondary Care; A&E, 111}.

Scanning paper documentation / receiving electronic communication, to the patients medical record.

Reading each document thoroughly.

Adding clinical codes (SNOMED) to the medical record.

Forwarding document to the relevant clinician for onward action.

  • You will build upon an excellent working knowledge of medical terminology.

  • You will work to a high standard to ensure that patient records are maintained accurately and that correct codes are used at all times.

  • Actioning LabLinks (electronic test results)

Receiving onto clinical system

Reading each result

Completing relevant template and file if necessary

Where results need to be actioned, forwarding to a clinician, according to the current practice policy.

  • Following current Practice Policy on receipt of Child Protection documentation.

  • Actioning deductions (where patients have died, or left the practice, notes to be pulled; electronic labels to be requested. Notes to be packaged securely and put ready for the PCSE Courier, who will transport them securely to PCSE.

  • Having an awareness of the GSF patients; and ensuring they are coded appropriately, and receive gold standard care.

  • To understand the importance of confidentiality.

  • To respond appropriately in emergency situations.

  • To report any incidents that might compromise health and safety for self or others.
