Job summary

We are a high-achieving, friendly and forward-thinking GP Training Practice, offering progressive general practice. Dynamic team of Partners that encourage special interests.

Excellent Multi-Disciplinary Team including ANPs, Clinical Pharmacists, and Paramedics, Urgent Care Practitioners, First Contact Physio, Mental Health Nurse.

The workload is very well managed, and the GPs are supported by an excellent clinical and clerical support team resulting in a well-run practice, a manageable work life and a healthy work life balance.

The practice premises are very modern and spacious, providing you with an excellent working environment. We are an accredited academic teaching practice, therefore very organised and there is lots of focus on the continuous training and development with opportunities for successful applicants to contribute to the development of the next generation of GPs.

Main duties of the job

Negotiable, to work between 6 to 8 sessions a week to suit individual need.

PERSONAL & CLINICAL RESPONSIBILITIES

Provide the full range of NHS Primary Medical Services to the patients registered at the practice, temporary residents and patients presenting for immediately necessary treatment including clinical examination, diagnosis, treatment and referral as appropriate.

Participate in Enhances services delivery.

Be personally accountable for professional and ethical actions and practices within a legal and ethical framework that adheres to the GMCs requirements and local policies and procedures.

Maintain confidentiality, while communicating patient information, in such a way that preserves the dignity and privacy of the patient and the patients family and carers.

Work in a non-judgemental anti-discriminatory way, with regard to the cultural and religious beliefs of individuals and groups.

Ensure that prior to any course of action involving individuals/groups, their informed consent or other valid authority has been obtained.

Promote and maintain optimum health of the registered patient population of the practice by identifying, planning and undertaking specific health promotional activities with identified individuals and target groups in order to improve their health and facilitate further service development.

Establish, develop and maintain appropriate systems for the management of patients with long-term conditions.

Ensure that record-keeping is managed through the creation and maintenance of concise, contemporaneous computerised records, and that all care is recorded accurately and in a timely fashion.

Interpret and explain complex facts in relation to a patients condition, taking appropriate action and giving advice in a professional manner to team members, and when appropriate to family and other carers, in such a way that protects and preserves the patients dignity.

Have continuing responsibility for Child Protection and Children in Need in all cases of suspected or confirmed abuse, and to ensure compliance with local and national guidelines, policies and procedures.

Ensure that concerns and identified potential risks are referred to the appropriate agency immediately.

Manage child protection cases efficiently and, when appropriate, to attend child protection supervision, to act accordingly to protect children at risk of harm and to have a clear knowledge and understanding of child protection issues.

Be responsible for appropriate referrals, ongoing support for the patient, family/carer and the co-ordination of other services.

Provide additional services and enhanced services within the post holders clinical competence.

Maintain clinical competence and exercise awareness of professional responsibilities by reading current relevant literature and by taking opportunities for personal and professional development in accordance with the requirements of the General Medical Council, recommendations of the Royal College of General Practitioners, National Health Service and Horsefair Practice Group policies.

Use evidence-based practice to develop and maintain high quality clinically effective and cost-effective primary care.

Participate in annual appraisal and, when implemented, fulfil the criteria for revalidation with the General Medical Council.

Maintain full registration with the General Medical Council at the employees own expense.

Assess and diagnose across a wide range of primary health care conditions, order investigations, plan and provide treatment as needed and discharge patients.

Assess, tolerate and manage risk and deal with uncertainty.

Prescribe in accordance with regulations and with the post holders training and registration and in accordance with CCG, and local formulary guidance.

Undertake screening procedures.

Provide telephone consultations as required.

Demonstrate critical thinking and diagnostic reasoning skills in clinical decision-making.

Ensure accurate completion of all necessary documentation associated with healthcare and practice administration.

Collaborate with other health professionals and agencies as appropriate, liaising and communicating effectively with medical, voluntary and statutory agencies as necessary.
