Job summary
We are seeking a dedicated and knowledgeable Clinical Pharmacist to join our multidisciplinary team to treat patients using their expert knowledge of medicines. The pharmacist will support and work alongside the practice team including a pharmacy technician for additional support.
The pharmacist will undertake clinical structured medication reviews to proactively manage patients including those with complex polypharmacy, potentially developing a specialist area of chronic disease or pain medicines management (such as hypertension/diabetes) The pharmacist will provide primary support to the practice staff for prescription and medication queries. They will support practice repeat prescription systems and lead medicines reconciliation on transfer of care ensuring safe systems for medicines. They will provide expertise in clinical medicines advice while addressing both public and social care medicines needs of patients in the GP practices.
This post is to cover the maternity leave of our existing clinical pharmacist and can be hybrid working.
Main duties of the job
- Conduct comprehensive medication reviews for patients, ensuring safety and efficacy of prescribed therapies.
- Collaborate with physicians and nursing staff to develop and implement patient-specific medication plans.
- Monitor patient responses to medications, adjusting treatment plans as necessary based on clinical assessments.
- Educate patients and their families about medication use, potential side effects, and adherence strategies.
- Maintain accurate records of medication administration and patient interactions in compliance with regulatory standards.

We are a training Practice with a growing list size of 7100 and dispense to approximately 40% of our patients. We work hard to maintain our excellent reputation in the community and enjoy a friendly atmosphere in the Surgery with our patients being known to many staff. We support a number of local nursing and learning disability homes and are actively engaged with fellow Practices in the Forest Green PCN. We pride ourselves on being an accessible service and have regularly scored top 3 in NHS surveys. We offer most of the Enhanced Services and join with the Forest Green PCN to provide EA sessions.

- Registration with the General Pharmaceutical Council (GPhC) or equivalent regulatory body.
- Excellent communication skills, with the ability to work effectively within a multidisciplinary team.
- A commitment to providing high-quality patient care and continuous professional development.
- Demonstrates use of appropriate communication to gain the co-operation of relevant stakeholders (including patients, senior and peer colleagues and other professionals, other, e.g. hospitals).
- Is able to recognise personal limitations, work to competencies, and refer to more appropriate colleagues when necessary.
- Explores the potential for collaborative working and takes opportunities to initiate and sustain such relationships.
