Job summary

Practice Nurse

We are looking for a dynamic qualified Nurse, preferably with Practice Nursing experience for 20 hours per week. You will be responsible to the Lead Practice Nurse, GP Partners and the Practice Manager.

The successful candidate should be able to work well within a team consisting of a range of different clinicians and should have the skills to be able to work autonomously to provide the required care for each individual patient. Practice nursing is a varied and flexible role that adapts to the ever-changing challenges of primary care. For further details please contact -

Main duties of the job

The ideal candidate should have experience of working within a Primary Care environment and be able to demonstrate experience of working within a team. They need to be able to work autonomously, if necessary. Experience in areas of Chronic Disease Management, Cervical Cytology and vaccinations/ immunisations is desired but not essential. We are a training practice & accommodate medical students & student nurses. We are keen to develop our nursing team & will give support where needed.

About us

Middleway Surgery is a busy practice providing care and support for approximately 6500 patients. We employ a full range of medical and clerical staff to provide the best possible care to our varied patient group. We are part of The Three Harbours/Bosvena Primary Care Network (PCN) within Bodmin, Fowey, Lostwithiel and St Blazey consisting of 4 other GP surgeries. Being part of a PCN enables us to share and gain knowledge and expertise between the different surgeries.

Within our surgery, our clinical team have 3 GP partners, 2 salaried GP. As we are a training surgery, we often also have Registrars, F2 Doctors & Medical Students. We have 1 Nurse Practitioners who provide both acute care and pre-booked appointments. Our nursing team currently consists of 1 Practice Nurses, 2 Health Care Assistants and a phlebotomist.

Job description

Job responsibilities

Please see attached Job Description for full information and Person Spec as required. Brief Outline:

The following are the core responsibilities of the practice nurse. There may be on occasion a requirement to carry out other tasks; this will be dependent on factors such as workload and staffing levels:

a. Develop, implement and embed health promotion and well-being programmes

b. Implement and evaluate individual treatment plans for chronic disease patients

c. Identify, manage and support patients at risk of developing long-term conditions, preventing adverse effects on patients health

d. Provide routine nursing care to patients as required in accordance with clinical-based evidence, NICE and the NSF

e. Provide wound care (ulcer/Doppler etc.) to patients

f. Provide travel medicine services

g. Request pathology services as necessary

h. Process pathology results as required

i. Provide chronic disease clinics, delivering patient care as necessary, referring patients to secondary/specialist care as required

j. Maintain accurate clinical records in conjunction with extant legislation

k. Ensure read codes/Snomed Codes are used effectively

l. Maintain chronic disease registers

m. Develop, implement and embed well woman clinics

n. Develop, implement and embed well man clinics

o. Chaperone patients where necessary

p. Assist GPs with minor surgery when required

q. Prioritise health issues and intervene appropriately

r. Support the team in dealing with clinical emergencies

s. Recognise, assess and refer patients presenting with mental health needs

t. Implement vaccination programmes for adults and children

u. Support patients in the use of their prescribed medicines or over-the-counter medicines (within own scope of practice)

v. Liaise with external services/agencies to ensure the patient is supported appropriately (vulnerable patients etc.)

w. Delegate clinical responsibilities appropriately (ensuring safe practice and that the task is within the scope of practice of the individual)

x. Support the clinical team with all safeguarding matters, in accordance with local and national policies

y. Understand practice and local policies for substance abuse and addictive behaviour, referring patients appropriately
