Within Primary Care we recognise that a lot of the patients that contact us looking for support have mental health or unexplained illnesses. We are looking to support our patients by providing a Mental Health Service within a multidisciplinary primary care setting.

The role aims to improve the experience for our patients with mental health needs who would have otherwise been seen by a GP.

This is an exciting opportunity to help develop a new role that sees the integration of the mental health services into the primary care team.

Post: Mental Health Nurse

Clinical Responsible to: GP Partners

Non-Clinically Responsible to: Practice Manager

Job Summary:

The post-holder will work as part of a team of Mental Health Nurses mostly based at Northdown Surgery but may be required to workg across 4 GP Surgeries providing a service for adults presenting with mental health problems/unexplained symptoms, they will provide specialist advice and provide support to their colleagues.

Purpose:

  • To work as part of the Urgent Care Team, triaging/screening and following up patients identified within the practice population.

  • To liaise with and refer onto secondary mental health services, voluntary and public sectors agencies and community support groups.

  • To work independently in the Surgery and the Community, providing assessment, treatment and referral of patients with specified mental health problems and unexplained symptoms.

  • The team will be working within a multi-professional primary care team to provide a service for adults with varying levels of mental health needs that may co-exist with physical health problems.

Clinical Responsibilities:

  • Communicate effectively with patients; recognising the need for varying methods of communication to overcome different levels of understanding, cultural background and preferred ways of communicating.

  • Work with other health and social care professionals to assess, treat and monitor the management of patients in line with locality needs. Attend multi-disciplinary meetings relating to referrals or patients in treatment, where appropriate.

  • Review medication for therapeutic effectiveness appropriate to patient needs and in according with evidence based practice and national and practice protocols.

  • Work with patients to support engagement with appropriate services and treatment, which may include onward referral to secondary mental health services, or sign-posting to other mental health services, voluntary services and support groups.

  • Educate and involve family members and others in treatment as necessary.

  • Keep detailed records of all clinical activity in line with service protocols on the GP record systems.

  • Undertake clinical audits of service performance, including service user surveys and evaluations, and help to collate and disseminate the results.

Professional Responsibilities:

  • Work as an effective and responsible primary care practice team member, supporting others and exploring the mechanisms to develop new ways of working.

  • Produce accurate, complete records of patient consultations, consistent with legislation, policies and procedures and in line with local practice.

  • Prioritise, organise and manage own workload in a manner that maintains and promotes quality.

  • Deliver care according to NICE guidelines and evidence-based care.

  • Actively participate in this new programme, evaluation and improvements to ensure benefits are realised.

  • Use practice computer systems accordingly in line with the practice policy.

  • Work effectively across the four surgeries, liaising with named leads appropriately.

  • Assess effectiveness of care delivery through self and peer review, and evaluation.

  • Collaborate on improvement the quality of health care in partnership with other clinical teams.

  • Support and participate in providing training and shared learning across the practice and wider organisation, including providing specialist advice and information relating to mental health.

  • Participate in the management of patient complaints when requested to do so and participate in the identification of any necessary learning brought about through clinical incidents and near-miss events.

  • Work within policies regarding safeguarding and refer as necessary.

  • Delegate clearly and appropriately, adopting the principals of safe practice and assessment competence.

  • Understand implemented internal and external referral mechanisms.

  • Prioritise own workload and ensure effective time management strategies are embedded within the culture of the team.

  • Take responsibility for own development, learning and performance including participating in clinical supervision and acting as a positive role model.

  • Develop contacts with and maintain a database of local knowledge on services, community support groups and social networks that can benefit patients.
