Within Primary Care we recognise that a lot of the patients that contact
us looking for support have mental health or unexplained illnesses. We are
looking to support our patients by providing a Mental Health Service within a
multidisciplinary primary care setting.
The role aims to improve the experience for our patients with mental
health needs who would have otherwise been seen by a GP.
This is an exciting opportunity to help develop a new role that sees the
integration of the mental health services into the primary care team.

Job Summary:
The post-holder will work as part of a team of Mental Health
Nurses mostly based at Northdown Surgery but may be required to workg across 4 GP Surgeries providing a service for adults presenting
with mental health problems/unexplained symptoms, they will provide specialist
advice and provide support to their colleagues.
Purpose:
To work as part of the Urgent Care Team,
triaging/screening and following up patients identified within the practice
population.
To liaise with and refer onto secondary mental
health services, voluntary and public sectors agencies and community support
groups.
To work independently in the Surgery and the
Community, providing assessment, treatment and referral of patients with
specified mental health problems and unexplained symptoms.
The team will be working within a
multi-professional primary care team to provide a service for adults with
varying levels of mental health needs that may co-exist with physical health
problems.
Clinical
Responsibilities:
Communicate effectively with patients;
recognising the need for varying methods of communication to overcome different
levels of understanding, cultural background and preferred ways of
communicating.
Work with other health and social care
professionals to assess, treat and monitor the management of patients in line
with locality needs. Attend
multi-disciplinary meetings relating to referrals or patients in treatment,
where appropriate.
Review medication for therapeutic effectiveness
appropriate to patient needs and in according with evidence based practice and
national and practice protocols.
Work with patients to support engagement with
appropriate services and treatment, which may include onward referral to
secondary mental health services, or sign-posting to other mental health
services, voluntary services and support groups.
Educate and involve family members and others in
treatment as necessary.
Keep detailed records of all clinical activity
in line with service protocols on the GP record systems.
Undertake clinical audits of service
performance, including service user surveys and evaluations, and help to collate
and disseminate the results.
Professional

Work as an effective and responsible primary
care practice team member, supporting others and exploring the mechanisms to
develop new ways of working.
Produce accurate, complete records of patient
consultations, consistent with legislation, policies and procedures and in line
with local practice.
Prioritise, organise and manage own workload in
a manner that maintains and promotes quality.
Deliver care according to NICE guidelines and
evidence-based care.
Actively participate in this new programme,
evaluation and improvements to ensure benefits are realised.
Use practice computer systems accordingly in
line with the practice policy.
Work effectively across the four surgeries,
liaising with named leads appropriately.
Assess effectiveness of care delivery through
self and peer review, and evaluation.
Collaborate on improvement the quality of health
care in partnership with other clinical teams.
Support and participate in providing training
and shared learning across the practice and wider organisation, including
providing specialist advice and information relating to mental health.
Participate in the management of patient
complaints when requested to do so and participate in the identification of any
necessary learning brought about through clinical incidents and near-miss
events.
Work within policies regarding safeguarding and
refer as necessary.
Delegate clearly and appropriately, adopting the
principals of safe practice and assessment competence.
Understand implemented internal and external
referral mechanisms.
Prioritise own workload and ensure effective
time management strategies are embedded within the culture of the team.
Take responsibility for own development,
learning and performance including participating in clinical supervision and
acting as a positive role model.
Develop contacts with and maintain a database of
local knowledge on services, community support groups and social networks that
can benefit patients.
Develop and maintain accessible information to
give to patients and their carers.
Contribute and participate in the development of
local guidelines, protocols and standards.
