Job summary
The Abingdon Surgery, a large GP
practice in Oxfordshire serving a patient population of 20,000, is seeking a
dedicated Clinical Pharmacist to join our expanding multidisciplinary team.
This is an exciting opportunity to play a pivotal role in improving patient
care, optimising medicines management, and enhancing the efficiency of our
prescribing processes.
The successful candidate will work closely with GPs, nurses, the Practice
Manager and other healthcare professionals, providing expert advice on medicines,
conducting medication reviews, and supporting chronic disease management.
The role also involves promoting safe prescribing practices and supporting
patient education regarding medication use.
This position is part of the Additional Roles Reimbursement Scheme (ARRS) and
offers significant professional development opportunities, including the
potential for independent prescribing qualifications and leadership roles
within the practice.
Main duties of the job
Main Duties of the Job
Conduct structured
medication reviews for patients with polypharmacy, long-term conditions, and
complex prescribing needs.
Provide expert
pharmaceutical advice to patients and the wider practice team, addressing
medication queries and ensuring safe and effective prescribing.
Manage repeat
prescribing systems, supporting medicine optimisation and reducing waste.
Process hospital
discharge summaries and outpatient letters, ensuring seamless medicines
reconciliation.
Support long-term
condition management, working within clinical guidelines for conditions such as
diabetes, hypertension, COPD, and cardiovascular disease.
Work collaboratively
with community pharmacists, hospital and ICB teams, nursing homes and PCN colleagues to improve medication safety and
continuity of care.
Support and deliver
quality improvement initiatives, including clinical audits and compliance with
prescribing best practices.
Participate in Care
Quality Commission (CQC) compliance and ensure adherence to local and national
policies on medicines management.
Provide training and
education to the practice team on prescribing updates, guidelines, and
medicines safety alerts.
