Due to pending retirement we are seeking enthusiastic General Practitioners to join our established team at SLC Medical Group. This is a great opportunity to join a supportive and patient committed team, with weekly clinical and practice meetings, a flexible approach to work patterns, manageable realistic workloads and clinical progression opportunities.
Our practices are based in Grimsby and Cleethorpes, North East Lincolnshire. The area has many attractions as a place to both work and live. The area is located on the edge of the Lincolnshire Wolds. As well as the larger towns the region has thriving market towns, picturesque villages, beaches, rolling hills, hidden valleys, and towns and villages that are not only steeped in history but offer affordable housing, outstanding schools and excellent transport links. The Lincolnshire Wolds is an area of outstanding natural beauty and is within easy reach of historic counties such as Derbyshire and Yorkshire. We have excellent road and rail links and some of the UKs most diverse and vibrant cities are easily accessible including Sheffield, Leeds and York.

Main duties of the job
The post-holder will undertake a
variety of duties including surgery consultations, telephone consultations and
queries, visiting patients at home, checking and signing off repeat
prescriptions and dealing with queries, paperwork and correspondence in an
appropriate timescale.

professional, autonomous decisions in relation to presenting problems, whether
self-referred or referred from other health care workers within the
organisation.
Assessing the health care needs of patients with undifferentiated and
undiagnosed problems.
Screening patients for disease risk factors and early signs of illness.
In consultation with patients and in line with current practice disease
management protocols, developing care plans for health.
Providing counselling and health education.
Admitting or discharging patients to and from the caseload and referring to
other care providers as appropriate.
Recording clear and contemporaneous consultation notes to agreed standards.
Collecting data for audit purposes.
Compiling and issuing computer-generated acute and repeat prescriptions.
Prescribing in accordance with the Northern Lincolnshire Area Prescribing
Formulary.
In general the post-holder will be expected to undertake all the normal duties
and responsibilities associated with a GP working within primary care.
