Job summary
We have a fantastic opportunity for a motivated Practice Nurse with qualifications and experience in Diabetes Management to join our friendly and dynamic team.
Main duties of the job
You will be running regular Diabetic clinics across the three practice sites as well as some routine vaccination and wound care clinics. If you have any other Practice Nurse qualifications such as smear taking or other chronic disease management we will ensure to utilise and support development of your skills and experience.

The post
holder is responsible for the delivery of Diabetes care to our patient population as well as other basic practice nursing duties. Supported by the clinical team within the
practice, they will deliver care within the boundaries of their role, focusing
upon supporting patients to be healthy, monitoring of long-term conditions,
health prevention and screening activities. They will work collaboratively with
the general practice team to meet the needs of patients, supporting the
delivery of policy and procedures, and providing nurse leadership and
mentorship as required. The post holder will also contribute to the practices
delivery of the Quality and Outcomes Framework (QOF).
