Job summary

As lead practice for the Gorleston Primary Care Network The Millwood Partnership is looking for a PCN Care Co-ordinator to support Gorleston PCN in delivering the PCN contract. This is a supportive role promoting health processes and intitiatives and helping to ensure that patients are able to access them. The role will develop as the contract changes from year to year.

Main duties of the job

You will need outstanding people skills, a caring and compassionate nature, be an excellent communicator and have a desire to learn in this ever changing environment

This is an exciting new role with scope to develop and deliver service improvements, working with the GP practices and wider community health teams.

Reception cover for our PCN colleagues will also be included in this job role.

About us

The Millwood Partnership is a forward thinking practice in Bradwell close to the east coast of Norfolk. It is lead practice for Gorleston Primary Care Network, with the Clinical Director and Management Lead both part of the practice team.

Clinically well-staffed we have 3 partners and 5 salaried GPs to cater for a diverse population of around 20,000 patients. The practice is led by the Partners and team of three managers, with a support staff including nurse practitioners, practice nurses, HCAs, pharmacists, paramedic, administration, prescription clerks and receptionists. We are lead practice for our Primary Care Network and are a teaching and training practice to medical students and GP Trainees respectively.

Job description

Job responsibilities

Key responsibilities

Undertake work in line with PCN directed priorities including reception duties at Falkland for your PCN colleagues

Proactively identify and work with a cohort of people to support their personalised care requirements

Ensure regular and consistent communication with the referrer regarding patient progress and any complications or guidance

Raise awareness of health promotion, screening and NHS health checks in practices

Support the evaluation of treatment programmes that promote health and well-being

Co-ordinate clinics, liaising with the schedulers and contacting the identified patients with appointments

Manage patient initiated calls for help/signposting etc., ensuring patients are directed to appropriate service

Document and monitor aspects of patient co-ordination and service delivery, supporting data collection and audit using the practices clinical system

Demonstrate the ability to recognise and respond appropriately when faced with a sudden deterioration or emergency situation, alerting the team or enabling a rapid response

Support national screening and immunisation programmes and encourage uptake

Monitor referrals to ensure tasks are completed and care delivered by keeping in regular telephone contact

Work closely with and refer to PCN social prescribing link workers were a patient is identified as potentially benefitting from this service

  • Diploma/ HNC level/ NVQ Level 2 in a health or social care related subject, or
  • Relevant experience in a similar role
  • Demonstrable commitment to professional and personal
  • development

  • Training in motivational coaching and interviewing or equivalent

Experience

Essential

  • Experience of working with healthcare professionals and or previous experience in the NHS or social care or relevant field (including unpaid work)
  • Experience of data collection and providing monitoring information to assess the impact of services
  • Experience of partnership/collaborative working and of building relationships across a variety of organisations
  • Experience of working with or in general practice
  • Working in a multi-disciplinary setting where influence and negotiation is required
  • Knowledge/familiarity with medical terminology
  • Experience of dealing with patients or the general public
