Job summary

As lead practice for the Gorleston Primary Care Network The Millwood Partnership is looking for a PCN Pharmacy Technician to to join the team in delivering the PCN contract.

This is a great opportunity for an experienced technician to be involved in direct patient care, expanding on their existing skills and seeing and helping patients within a different context.

Main duties of the job

Working as part of the network team, you would be joining fellow technicians to support medicines management and prescribing initiatives in the practice and care homes as well as potential to work with our visiting teams for housebound patients and liaising with community pharmacy.

This is an exciting new role with scope to develop and deliver service improvements, working with the GP practices and wider community health and voluntary teams.

About us

The Millwood Partnership is a forward thinking practice in Bradwell close to the east coast of Norfolk. It is lead practice for Gorleston Primary Care Network, with the Clinical Director and Management Lead both part of the practice team.

The growing network team is made up of a pharmacist and two other technicians, first contact physio service, mental health worker, social prescribers and care coordinators.

Clinically well-staffed we have 9 partners and 2 salaried GPs to cater for a diverse population of around 19,000 patients. The practice is led by the Partners and team of three managers, with a support staff including nurse practitioners, practice nurses, HCAs, pharmacists, paramedics, administration, prescription clerks and receptionists. We are lead practice for our Primary Care Network and are a teaching and training practice to medical students and GP Trainees respectively.

Job responsibilities

Clinical responsibilities of the Pharmacy Technician

Undertake patient-facing and patient-supporting roles to ensure effective medicines use, through shared-decision making conversations with patients.

Carry out medicines optimisation tasks including effective medicine administration (e.g. checking inhaler technique), supporting medication reviews, and medicines reconciliation. Where required, utilise consultation skills to work in partnership with patients to ensure they use their medicines effectively.

Support, as determined by the PCN, medication reviews and medicines reconciliation for care home patients and synchronising medicines for patient transfers between care settings and linking with local community pharmacists.

Provide specialist expertise, where competent, to address both the public health and social care needs of patients, including lifestyle advice, service information, and help in tackling local health inequalities.

Take a central role in the clinical aspects of shared care protocols and liaising with specialist pharmacists for more complex patients.

Support initiatives for antimicrobial stewardship to reduce inappropriate antibiotic prescribing.

Assist in the delivery of medicines optimisation and management incentive schemes and patient safety audits.

Support the implementation of national prescribing policies and guidance within GP practices, care homes and other primary care settings. This will be achieved through undertaking clinical audits (e.g. use of antibiotics), supporting quality improvement measures and contributing to the Quality and Outcomes Framework and enhanced services.

Technical and Administrative responsibilities

Work with the PCN multi-disciplinary team to ensure efficient medicines optimisation.

Supervise the prescribing and practice reception teams in sorting and streaming general prescription requests, so as to allow GPs and Clinical Pharmacists to review the more clinically complex requests;

Provide leadership for medicines optimisation systems across PCNs, supporting practices with a range of services to get the best value from medicines by encouraging and implementing Electronic Prescriptions, safe repeat prescribing systems, and timely monitoring and management of high-risk medicines;

Provide training and support on the legal, safe and secure handling of medicines, including the implementation of the Electronic Prescription Service (EPS); and

Develop relationships with other pharmacy technicians, pharmacists and members of the multi-disciplinary team to support integration of the pharmacy team across health and social care including primary care, community pharmacy, secondary care, and mental health.

Medicines management

Dealing with medication queries including acute medication requests, patient medication and prescription queries. This may involve removing unwanted items, checking review dates, ensuring necessary blood tests and monitoring has been carried out, prescription synchronization etc.

Updating of medication received from secondary care on the patient medical record, where appropriate.

To support the lead GP for prescribing with clinical administration, projects and audits to improve services and initiatives to enhance safety, quality and cost effectiveness of prescribing (e.g. Prescribing Quality Scheme as assigned by CCG Medicines Management Team, MHRA updates etc.)
