Lea Vale Medical Group are seeking a dedicated Advanced Nurse Practitioner to join our outstanding CQC-rated practice. In this role, you will work autonomously while being part of a supportive multidisciplinary team.
We are looking for someone to work part time with a minimum of 15 hours per week.
Interview Dates will be week beginning 18th of August and will take place for 1/2 a day.
Main duties of the job
You will play a key role in delivering expert clinical care including, minor illness, chronic disease management and health promotion while making a real difference to the health of our community. 1
Clinical Care
- Assess,
plan, develop, implement, and evaluate programmes to promote health and well
being.
- Liaise
and network closely with other colleagues within the Practice and with other Primary
Care professionals, referring patients as necessary.
- Ensure
evidence based care is delivered at a high standard to patients providing expert
clinical advice as appropriate.
