Job summary
Lea Vale Medical Group have an exciting opportunity for a Patient Navigator to join our surgery. This is an exciting role as you'd be joining an established non-clinical team that serves just over 40,000 patients and is rated outstanding by the CQC.
Both full time and part time hours will be considered.

Main duties of the job
Our Patient Navigators provide an important link between the general public and the Practice. The role involves using our online consultation tool to triage patient enquiries and signpost or book appointments as appropriate. The role will also include responding to other patient enquiries as necessary.

Patient Navigator Duties
- Initiating contact with and responding to requests from patients, other team members and associated healthcare agencies and providers
- Assist and direct patients in accessing the appropriate service or healthcare professional in a courteous, efficient and effective way.
- Provide patients, relatives and other services with correct information
- Operate clinical computer systems and maintain high quality data entry.
- Communicate with other agencies like transport, phlebotomy, hospital etc.
Maintenance of Records
- Accurately record details
- Make sure patient information is entered accurately and stored confidentially.
- Record on the SystmOne system on daily basis outgoing prescriptions.
- Carry out range of clerical duties, as required

- Good level of education to GCSE standard or equivalent
Experience

- Experience of working in a customer services/other front line situation
- Computer literate with excellent keyboard skills
- Self-starter, able to work on own initiative, following set procedures and protocol
Desirable
- Use of clinical system software eg. Systmone
- Previous NHS experience ideally within a GP setting
Skills and Abilities

- Advanced working knowledge of Office 365 applications
- Excellent communicator both verbally and written demonstrated with people at all levels and in all circumstances
- An understanding, acceptance and adherence to the need for strict confidentiality
- Can plan own work and multi-task
- Able to accept and deal with a heavy workload

- An enquiring mind with a willingness to learn new skills and procedures
