Job summary
An exciting opportunity has arisen for a Health and Wellbeing Coach to join our multidisciplinary team at Westrop Medical Practice. This role is for 15 hours per week and is integral to our innovative Metabolic Health Clinic, which focuses on empowering patients with metabolic symptoms, including Type 2 diabetes, to achieve sustainable dietary changes and improve their condition without sole reliance on medication.
As a Health and Wellbeing Coach, you will be crucial in delivering tailored interventions that address the root causes of metabolic health issues, such as poor nutrition, lack of physical activity, stress, and unhealthy lifestyle habits. This role is ideal for individuals passionate about empowering patients to take control of their health and improve their quality of life.
You will work closely with our lead GP as part of the metabolic team and contribute to the further development of this service.
Main duties of the job
Personalised Coaching:
Provide one-on-one coaching to patients, focusing on improving metabolic health
through goal setting for nutrition, exercise, stress management, and other
lifestyle factors.
Motivational Interviewing:
Utilise evidence-based techniques like motivational interviewing to help
patients overcome barriers to behavioural change and foster a collaborative
approach to health management.
Collaboration: Work
closely with colleagues in the Metabolic Health Clinic and across Westrop
Medical Practice for comprehensive, seamless, holistic, and integrated patient
care.
Metabolic Health Education:
Educate patients on key aspects of metabolic health, including the role of
diet, exercise, and sleep in managing conditions like diabetes and obesity.
Outcome Monitoring: Track
and report patient progress via regular check-ins and follow-up, adjusting
interventions to ensure optimal outcomes.
Support Groups: Lead or
co-facilitate group sessions, including for newly diagnosed prediabetics, to
create a supportive environment for sharing experiences and strategies for
managing metabolic conditions.
Patient Advocacy: Advocate
for patients within the clinic, ensuring support throughout their health
journey and encouraging active participation in care plans.
Service Development:
Participate in Metabolic Health Clinic planning meetings and review patients at
2-3 month, 6 month, and 12 month intervals.
