Job summary

As part of the NHS England Additional Roles Reimbursement Scheme (ARRS), we are seeking an experienced and enthusiastic Practice Nurse to join our Primary Care Network (PCN) team. The role is designed to support the transformation of primary care and enhance patient access, health outcomes, and collaborative working across member practices.

The required working hours for this role will be 35.

Main duties of the job

The Practice Nurse is expected to provide practice nursing services to patients and work as part of a multidisciplinary team delivering care in accordance with clinical based evidence; NICE and the NSF, identifying, managing and supporting patients.

The successful candidate will need to deliver their nursing duties and be responsible for a number of clinical areas such as treatment room duties, long term condition management, travel health, health prevention and promotion, immunisations, wound care, clinical audits etc.

General

  • Work with nursing colleagues, GPs and other professionals to develop quality standards, protocols/guidelines/patient group directions to ensure safe, appropriate and effective care
  • Work within the latest NMC Code of Professional Conduct
  • Ensure the delivery of nursing care is evidence based and own practice is clinically reviewed through supervision and reflective practice as part of revalidation
  • Provide clinical support to the nursing team and promote collaborative working relationships between all members of the multi-disciplinary team.
  • Contribute to clinical/staff meetings professionally and effectively
  • Achieve targets set thus contributing to the profitability of the practice, by timely completion of paperwork
  • Manage and control effective and economic use and ordering of stock, dressings, drugs etc. monitoring usage and costs

The Act requires the post holder to attend infection prevention and control training on induction and at regular updates and to take responsibility for the practical application of the training in the course of their work. Infection prevention and control must be included in any personal development plan or appraisal.

  • Utilise the EMIS Clinical System to include population manager
  • Apply the principles of read coding and have the ability to descend through code levels to reach most accurate read code level for diagnosis, operation, disease etc. Understand the relevance of read coding to searching
  • Use of systems within the practice including docman, EMIS, outlook/email, telephones, internet
