Lion Health is a large, friendly teaching practice serving a population of approximately 28,500 patients.
We are based in a modern, recently renovated premises in central Stourbridge.
This post is for a permanent ANP/ACP who will work 37.5 hours/week. However, part time positions can be considered.
Main duties of the job
We are seeking a skilled and motivated Advanced Nurse Practitioner (ANP)
/ Advanced Clinical Practitioner (ACP) to join our dynamic team in a General
Practice setting. The successful candidate will be an experienced healthcare
professional with advanced clinical skills, providing high-quality,
patient-centred care across a range of services, including but not limited to
minor illness, chronic disease management, and acute presentations. They will
be an experienced non-medical prescriber who can work autonomously, with
support from the medical team.
As an ANP/ACP, you will be an integral part of the multidisciplinary
team, helping to improve patient outcomes and access to primary care services.
You will be responsible for assessing, diagnosing, and treating patients,
providing clinical leadership, and working collaboratively with the wider team.

Job responsibilities
Clinical Assessment and Care
- Provide expert clinical assessment, diagnosis,
and treatment for a wide range of acute and chronic
conditions, within the scope of practice.
- Conduct
comprehensive health assessments for patients with complex care needs and
those with multiple long-term conditions.
- Prescribe
medications and interventions in accordance with clinical guidelines and
scope of practice.
- Undertake
home visits as needed to assess and support patients in their own
environment.
Patient Management
- Develop,
implement, and evaluate patient management plans in collaboration with
patients, their families, and the multidisciplinary team.
- Deliver
safe, evidence-based, and patient-centred care, ensuring that the
individuals needs are met.
- Provide
follow-up care and support, monitoring progress and adapting treatment
plans where necessary.
Leadership and Collaboration
- Provide
clinical leadership and mentoring to junior colleagues and other members
of the clinical team.
- Work
closely with GPs, nurses, and other healthcare professionals to ensure
integrated care delivery.
- Promote
best practices in clinical care and contribute to continuous professional
development and team learning.
Quality and Compliance
- Participate
in audits, reviews, and quality improvement initiatives within the
practice.

that all clinical activities comply with relevant policies, procedures,
and regulations

will actively participate in the prevention and control of infection within the
capacity of their role. The Act requires
the post holder to attend infection prevention and control training on
induction and at regular updates and to take responsibility for the practical
application of the training in the course of their work. Infection prevention
and control must be included in any personal development plan or appraisal.
