Job summary
Hatfield Health Centre wants to recruit a key member of the practice team. Working along another clinical pharmacist and a pharmacy technician, you will provide expert advice and training around medications for our patients and the other clinicians and non-clinicians in the practice team. We are a practice which respects and encourages the role that clinical pharmacists play in providing a safe medication service for our patients. We have a record of being high achievers in local Medicine Management schemes and other quality improvement initiatives. The practice shares a site with an independently owned pharmacy and we enjoy a productive, collaborative relationship with the pharmacy, for our patients, who choose to use it.
Main duties of the job
The main responsibilities of the role are to:
Review patients by face to face and remoted consultation on complex medication
regimens, screening & management of long term conditions, such as diabetes,
asthma, COPD & hypertension
Deal with telephone based medication queries from patients,
pharmacies & hospital
Develop and utilise prescribing skills in face to face and
remote settings to further support patient care.
Provide medicines management advice to doctors, practice
nurses and other health professionals to promote the safe, effective, quality
and cost effective use of medicines
Run audits & design of safer prescribing systems
Review of medications for nursing & residential home
patients
Manage patients & health care professionals medicine
queries

About us
We are based in a modern, purpose built building in the village of Hatfield. We have over 9700 patients and provide a range of medical services to the local community. The practice is one of the main hubs of Hatfield. We share our premises with and independent pharmacy and Community Nursing from RDASH, so it is helpful to have primary care colleagues we can collaborate with on site.
We are a friendly team who are passionate about providing high quality care and services to our patients.
Our Practice comprises:
5 GP Partners, 3 Salaried GPs, Physician's Assistant, 5 Practice Nurses, 2 Clinical Pharmacists, Health Care Assistants, Nurse Associates, Physiotherapists, Mental Health Nurses, receptionists and admin support staff, Reception Supervisor, Operations Manager and Business Manager.

- Review and process acute prescription requests, ensuring safe and evidence-based prescribing.
- Action and manage hospital discharge summaries, outpatient letters and other clinical correspondence.
- Provide medicines optimisation support, including deprescribing, monitoring, and safety interventions.
- Support chronic disease reviews as part of the multidisciplinary team (e.g., hypertension, asthma, diabetes, heart failure).
- Manage patient queries relating to medicines, minor ailments and condition self-management.
- Provide prescribing advice and guidance to practice teams within your clinical competence.
