Job summary
We are seeking a fixed-term Pharmacist to work in our Prescribing Hub as part of a friendly and dedicated team of Clinical Pharmacists, Pharmacy Technicians and Prescribing Clerks, providing services to two GP practices in a collaborative Primary Care Network of about 33,000 patients. Our ideal candidate will likely work 3 days per week with the exact working pattern to be confirmed.

Main duties of the job
The postholder will play a key role in the delivery of high-quality primary health care within Northern Arc PCN. Working within their clinical competencies, they will provide specialist pharmaceutical support to the practices of the PCN . This will include working with members of the wider healthcare team to improve care and support safe and effective prescribing.
The post holder will contribute to the work of the Prescribing Hub, including supporting and assisting the Pharmacy Technicians. The role will have a patient facing element to include face to face, or telephone consultations - including but not limited to chronic disease management, repeat prescription management and structured medication reviews. This will also include the management and review of patients with polypharmacy, care home and hospital discharge summaries.
The post holder will be supported to help develop the role and work to improve the care and outcomes for the patients across Northern Arc PCN.

Qualifications
Essential
- Degree in pharmacy, 4-year programme leading to MPharm, BPharm or BSc -Pharmacy- together with equivalent experience
- One year pre-registration experience pre-registration examination passed and tutor assessment satisfactory
- Current registration with the General Pharmaceutical Council
- Experience as a band 6 (or equivalent) qualified pharmacist
- Completed CPPE Primary Care Education Pathway, OR currently enrolled OR willing to enrol
