Job summary

Job Summary

The post holder is responsible for the delivery of basic practice nursing services care to the practice population. Supported by the partners and nursing team within the practice, they will deliver care within the boundaries of their role, focusing upon supporting patients to be healthy, monitoring of long-term conditions, health prevention and screening activities. They will work collaboratively with the general practice team to meet the needs of patients, supporting the delivery of policy and procedures, and providing nurse leadership as required.

Main duties of the job

Role/Job Summary:

  • To practise a range of clinical skills gained through previous experience, undertaking the delivery of nursing care as a self-directing practitioner, working with the GPs to meet the clinical needs of our patients as appropriate.
  • To support structures already in place for the smooth running of the practice.
  • To provide effective leadership and management skills.
  • To ensure own actions are consistent with clinical governance systems.
  • To conduct audit and patient screening in pursuance of the objectives of the Quality & Outcomes Framework (QOF) of the PMS contract.

  • Membership of a professional body
  • Clinical skills cervical cytology, immunisation and vaccination, ear care, minor surgery
  • Change-management skills and ability to support patients to change lifestyle
  • Communication skills, both written and verbal
  • Ability to communicate difficult messages to patients and families
  • Negotiation and conflict management skills
  • IT skills

Desirable

  • Practice Nursing course
  • Chronic Disease Management Qualification
  • Two or more years relevant experience in a patient centred setting
  • Uses initiative
