Job summary
Windsor House Group Practice is a large GP Practice with 3 x surgeries based in Leeds (Morley, Churwell and Drighlington) with a list size of over 18,500 patients.
The post holder is a clinical pharmacist who acts within
their professional boundaries, working alongside a team of pharmacists in
general practice.
They will improve patients health outcomes and the
efficiency of the primary care team by providing direct, accessible and timely
medicines and expertise including face to face contact with patients.
The post holder will be an integral part of the general
practice team, as well as part of a wider pharmacist network and
multi-disciplinary team. This includes being a conduit of patient medicines
information into and out of hospitals. They will work to optimise medication issues to improve patient care
and safety, and support clinical staff in the management of patients.
As part of their employment, the post holder will achieve
qualifications from an accredited training pathway, including independent
prescribing, equipping the clinical pharmacist to be able to practise and
prescribe safely and effectively in a primary care setting.
The supervision and mentorship network will be
provided to support the clinical pharmacist.
Main duties of the job
The successful candidate will have a patient facing role in managing long-term conditions; working directly with patients to assess and treat conditions, as well as promote self-care.
The candidate will have excellent written and verbal communication skills, experience of influencing others and conducting clinical medication reviews.
The successful applicant will work as part of a multidisciplinary team to develop and run (for example processes for repeat prescription reauthorisation, management of medicines on transfer of care and systems for safer prescribing). They will work with and be supported by members of the MDT including pharmacists and pharmacy technicians from the GP Confederation.
In return, we will support them to develop their role, including, for example, becoming an independent prescriber or gaining a postgraduate qualification in clinical pharmacy.
About us
Windsor House Group Practice is a large GP Practice in Leeds spanning across three sites. We have a team of approx 60 employees both clinical and administrative. We are passionate around the care and support of our patients and the role of a receptionist is key to our service provision.
The role will primarily work at Adwalton House Surgery, Drighlington (with an expectation that the successful candidate would work at one of the other surgeries as and when required).

- Mandatory registration with the General Pharmaceutical Council
Desirable
- Membership Primary Care Pharmacy Association (PCPA)
- Membership of the Royal Pharmaceutical Society
- May hold or be working towards an independent prescribing qualification
- Clinical Diploma or equivalent (demonstrate experience)

- Minimum of two years experience as a pharmacist, demonstrated within a practice portfolio.
- Experience and an awareness of the breadth of common acute and long-terms conditions that are likely to be seen in general practice.
- Demonstrates ability to integrate general practice with community and hospital pharmacy teams community groups.
- Experience of partnership/collaborative working and of building relationships across a variety of organisations.
