Job summary

Morley and District Primary Care Network (PCN) is a forward thinking and dynamic group of 7 practices with 62,000 patients.

Main duties of the job

This role will involve dealing with referrals from practices and patients directly, ensuring that you target groups and communities most in need and then supporting them to access help.

Social Prescribers will support patients to live independently within the community following discharge from hospital, connecting people to groups and services for practical and emotional support.

You will achieve this by working alongside the NHS providers and community social care providers as well as networking with other local voluntary organisations in a truly multi-agency approach. We value your input and we hope you will identify areas for improvement in the service that is provided, ensuring that it is utilised to the full within the community.

Job responsibilities

To deliver a national service specification

The main purpose of the role being to deliver social prescribing support to the Morley and District PCN as required by the PCN Board.

Job Summary

The social prescriber will, working in partnership with the practice teams, lead on projects such as supporting patients by promoting lifestyle changes, preventing ill-health by means of behavioural change techniques etc. The social prescriber may work with patients in the practice or with patients in their own home.

Practice Leaders (Partners and Practice Managers)

Clinical Commissioning Group (CCG) and NHSE

Responsibilities underpinning the role

To assist the PCN Director(s) and board in setting and realising the PCN vision, mission and business strategy

To play a role in the delivery of high-quality primary health care services

Provide support to individuals, their families and carers to take control of their health and wellbeing, live independently and improve their health outcomes, as a key member of the PCN multi-disciplinary team.

Working under supervision of the core network member practices i.e. the GPs, practice managers and the Business Managers take referrals from the network practices.

To make and maintain contact with local health and wellbeing service providers and build a network and knowledge of referral routes to and from service providers, keeping a Directory of Service updated.

To represent the PCN in cross organisation meetings when agreed e.g. locality/CCG meetings

Proactively identify and arrange locality-based wellbeing events to improve the general health of patients in the Network.

Co-ordinate training and development of PCN clinical team

Collate feedback / analysis data on behalf of the PCN to report to the PCN Board and CCG/NHSE as required
