Job summary

The Personalised Care Team Co-ordinator is a new, part-time role, however we are flexible on working and will consider job share for the right candidates or increased hours. The role can be hybrid, working of any of the three main practices when in the office.

  • Distribute self-referrals to the team to convert to appointments.
  • Up-date the PCN website (training will be given)

Main duties of the job

The main responsibilities include, but are not limited to:

  • Monitoring and inputting patient satisfaction surveys
  • Setting up sessions, planners and templates for Team Appointments.
  • Scheduling of meetings for the larger team and Board including compiling papers and taking minutes.
  • Work with the Practice Managers on events and other ad hoc administration.

About us

You will be employed by Healthier Oxford City Primary Care Network (PCN Our growing PCN covers 4 local practices; Banbury Road Medical Centre , 19 Beaumont Street Surgery, Luther Street Medical Practice and Summertown Health Centre (incorporating Cutteslowe and Wolvercote Surgeries) and provides Social Prescribing Link Workers, Health Coaches, Care Co-Ordinators, Clinical Pharmacists, First Contact Physiotherapist and Mental Health Professionals over all the practices. We are a small organisation with big ideas on the need for; patient education, providing proactive care, freeing up Clinicians time and ensuring our patients get an appointment with the right practitioner in a timely manner.

Our primary aim is to recruit the right type of person to the role, a can-do, professional, and caring attitude will be advantageous.
