Job summary
The Personalised Care Team Co-ordinator is a new, full time, role, however we are flexible on working and will consider job share for the right candidates.
Practices in the PCN to convert referrals to appointments for our personalised care team and balance utilisation.
Main duties of the job
The main responsibilities include, but are not limited to:
- Work with the GPs and other primary care professionals within the PCN to identify and manage a caseload of patients, and where required and as appropriate, refer people back to other health professionals within the PCN
- Raise awareness within the PCN of shared decision-making and decision support tools
- Raise awareness of how to identify patients who may benefit from shared decision making and support PCN staff and patients to be more prepared to have shared decision-making conversations
- Making timely and suitable appoints, from referrals, for the PCN larger team.
- Compiling reports in order to assess utilisation
- Monitoring and inputting patient satisfaction surveys
- Scheduling for the larger team
- Work with the PCN Manager on events other ad hoc administration.
We are looking a great administrator with a caring attitude and exceptional communication and organisation skills, including an ability to prioritise, multi-task and work to deadlines. Experience
In GP surgery and/or hospital/medical services and previous experience with EMIS will be an advantage. However, these are not essential as full training will be provided. Our primary aim is to recruit the right type of person to the role, a can-do, professional, and caring attitude will be advantageous.

You will be employed by Healthier Oxford City Primary Care Network (PCN Our growing PCN covers 4 local practices; Banbury Road Medical Centre , 19 Beaumont Street Surgery, Luther Street Medical Practice and Summertown Health Centre (incorporating Cutteslowe and Wolvercote Surgeries) and provides Social Prescribing Link Workers, Health Coaches, Care Co-Ordinators, Clinical Pharmacists, First Contact Physiotherapist and Mental Health Professionals over all the practices. We are a small organisation with big ideas on the need for; patient education, providing proactive care, freeing up Clinicians time and ensuring our patients get an appointment with the right practitioner in a timely manner.
