Job summary
An opportunity to join East Norfolk Medical Practice's award winning (RCGP Practice Team 2019) Urgent Care Team (UCT) is available. The first team of its kind in Norfolk and Waveney, the Team of eleven goes from strength to strength. It is now made up of six paramedics, three physician associates and two advanced nurse practitioners.
The role of the Team is to work alongside our significant GP team of eighteen GPs, to provide routine and urgent care to patients. Team members have a varied role. Typically, in the morning they work alongside the GP they support to provide routine primary care to the GP's patients. At ENMP each GP is responsible for a personal list of patients to maximise continuity of care. In the afternoon they will work in the UCT to provide care to those patients who wish to be seen urgently either at the Practice or via home visits including care home visits, if appropriate. The UCT each afternoon works alongside and is supported by two duty doctors.
Mentorship and supervision are provided by the GP or GPs the post holder works with. The level and type will be dependent on the post holder's experience and competence and in line with the organisation's clinical governance arrangements.
The successful applicant will have six weeks holiday per year and one weeks paid study leave together with a personal study budget. They will also receive free staff parking and won't have to work on their birthday!!
Main duties of the job
The role will include initial history taking, clinical assessment, appropriate onward referral, diagnosis, treatment and evaluation of patient care, demonstrating safe clinical decision-making. The postholder will work collaboratively with UCT members and the wider healthcare team to meet the patients needs initiating and maintaining agreed management plans according to local and national protocols.
The role may require completion of home visits and some care home visits.
About us
ENMP is a large general practice with four sites from where we care for over 32,000 patients. Our practice area cares for a wide demographic as we cover urban areas, including one of the most deprived wards in the country to a retired village population. ENMP also holds the contract to provide enhanced care to the area's homeless population.
We have eighteen GPs, seventeen of them partners, so there is always support available to members of the UCT. Unlike in many practices, this team does not exist to replace GPs but to work alongside them.
At ENMP we work hard to provide outstanding care to our patients but we do not work in isolation. Apart from the day to day support from GPs, the UCT meet each day for breaks and meet once a fortnight for an educational meeting with one of the GPs. The ability to continually learn and develop is a key foundation for our UCT. Three of the team joined as non prescribers but have been supported to obtain their prescribing qualification. Others have or are being supported to undertake leadership, long term conditions, sexual health or mental health qualifications.
The UCT members also benefit from regular debriefs with GPs and supportive one to one's with the Managing Partner, not to mention the Christmas and summer parties!!

The post holder, acting within their professional boundaries, will provide care for the personal list of patients registered with their Accountable GP, primarily through face to face clinical consultation. The post holder will also carry out similar work as part of the Practices Urgent Care Team, dealing with a variety of patients, not just those of the Accountable GP.
They will work collaboratively with the Practice and wider healthcare team to meet the needs of their patients. The post holder will provide a holistic clinical service, with support from the Accountable GP and where appropriate other GPs as required, implementing agreed management plans and following approved protocols. The post holder will be expected to consult with patients who present on the day and via booked appointments and to carry out some home visits. The post holder will also review and act appropriately on blood results and referrals/correspondence regarding their patients.
Clinical Practice
Utilising advanced clinical assessment skills autonomously assess, diagnose and treat a wide range of conditions and presentations.
Request further tests, such as blood tests, imaging and ECGs, to further aid the diagnosis of complex conditions.
Utilise various consultation techniques, ranging from in-person consultations through to remote assessments, utilising technology such video, telephone and picture- based techniques.
Carry out visits to housebound patients and those resident in care homes.
Interpretation of blood results and acting upon these findings.
Referral of patients to colleagues, either within primary or secondary care.
Recognising the limitations of knowledge and clinical skills and seeking further advice to ensure a positive patient outcome.
Facilitating Learning
Mentor and teach students who are on placement within the Practice.
Aid junior staff in their practice and facilitate learning to enhance their clinical skills.
Reflect upon practice, both positive and negative, for the benefit of personal and professional growth.
Participate in bi-weekly teaching.
Leadership
Act as a leader within the Practice and a beacon for clinical excellence.
Utilise different leadership styles and skills to guide staff and patients.
