Practice Nurse (30 - 35 hours per week)

This is an exciting opportunity for a committed and enthusiastic Practice Nurse to join our team.

To be responsible for management of patients with long-term conditions using evidence-based practice as key member of multidisciplinary team. To provide the full range of nursing procedures as included in this Job Description.

The Practice Nurse will be responsible for a number of clinical areas such as infection prevention and control, chronic disease management, health promotion, well women and NHS Health Checks clinics, as well as actively supporting the practice management team in the reviewing and delivery of clinical policy and procedure, QOF and IIF targets.

The role holder will support, train and develop the Health Care Assistants within the team.

Main duties of the job

About us

Malpas Surgery is a rural dispensing practice of 7,800 patients. We have excellent management and administration to support the Practice Nursing team which currently comprises 3 Health Care Assistants and 2 other Practice Nurses. The wider clinical team includes a Clinical Pharmacist and 7 GPs.

We are within easy reach of Chester, Nantwich, Whitchurch and Wrexham. Informal visits are welcome.
