Job summary
Malpas Surgery is seeking to recruit a Full Time (37.5 hours per week) Care Coordinator, to support achievement of both Practice and Primary Care Network objectives.
As a member of the Rural Alliance Primary Care Network, the successful post-holder will join a team that works across the six rural practices of Malpas Surgery, Tarporley Campbell & Partners, Tarporley Adey & Dancy, Bunbury Medical Practice, Kelsall Medical Centre and Village Surgeries Group.
The Care Coordinator will have a broad portfolio of duties that originate from the requirements of the PCN DES and PCN IIF and will contribute to better patient care by supporting practice recall (including QOF), safeguarding and wider administrative processes.
This is the ideal opportunity for someone already working in General Practice who is looking to take the next step in their Primary Care career. However, full training will be given to the right candidate from a related background.

Main duties of the job
As a senior administrative role, you will have the skills to work with colleagues across the Multi-Disciplinary Team, with excellent attention to detail and strong IT skills.
You will be expected to work during the surgery opening hours, between 7.45am and 6.30pm and be flexible to travel as required between Practice sites in the Primary Care Network.
