Whitby Group Practice
are looking for a highly motivated, experienced Advanced Clinical Practitioner
to join our well-established clinical team for up to 37.5 hours per week.
In this role you will work independently, making professional,
autonomous decisions.
The role will involve initial history taking,
clinical assessment, diagnosis, treatment plan and evaluation of patient care.
Assessment may be face to face by appointment or by telephone as part of a
triage/unscheduled care team and will include patients presenting with an
undifferentiated diagnosis.
You will be responsible for making any necessary
referrals, requests for investigations and do these in in an appropriate manner
and timescale.

About us
Whitby Group Practice is a personal Medical Services Practice,
located in Spring Vale Medical Centre in Whitby, North Yorkshire. It has a
current list size of 13,800 patients.
The Practice provides a service to residents of Whitby and the
surrounding villages. It has a branch surgery at Robin Hood’s Bay,
approximately 5 miles from Whitby along the coast. Both surgeries are purpose
built, have a dispensary and ample parking. They are owned by the Partners.
The Practice is a large team which works together to deliver
high quality, accessible care within a supportive, friendly learning
environment. The GP Partners have personal patient lists, and most work
part-time work in a job-share team with other Partners.

Clinical practice
- Assess,
diagnose, plan, implement and evaluate treatment/interventions and care
for patients presenting with an undifferentiated diagnosis
- Clinically
examine and assess patient needs from a physiological and psychological
perspective, and plan clinical care accordingly
- Carry
out face to face, telephone, video appointments and triage patients
appropriately
