Job summary

Diabetic / Practice Nurse

30 Hours per Week

(Monday to Friday (Working Days Negotiable (Some Days fixed to fit with nursing rota)))

We are looking to appoint an enthusiastic, highly motivated nurse to join our nursing team. The successful candidate will have experience working as a diabetic nurse.

The role will also include working as a practice nurse, duties to include immunisations, cervical cytology, chronic disease management, travel advice and wound care, training will be provided if necessary. There will also be the opportunity to develop skills in research.

The role will predominantly involve the care of diabetic patients within planned diabetic clinics.

Salary will be dependent on qualifications and experience with the opportunity to join the NHS Pension Scheme - WTE £33,000 - £36,000 per annum

Main duties of the job

The post holder will be an RGN with current NMC registration. They will also require a diabetic qualification and experience working as a diabetic nurse.

An ability to work independently and autonomously is essential. Support will be provided by the GP lead for diabetes.

The practice will support the successful candidate in developing their skills as required.

JOB DESCRIPTION

POSITION: Practice/Diabetic Nurse

ROLE: To provide effective nursing services for the practice population.

ACCOUNTABLE TO: The partners and the NMC

RESPONSIBLE TO: Dr R Clark/Dr George & Practice Manager

KEY TASKS

Diabetes Management

1) Identification and diagnosis of Diabetes

2) Initial explanation about Diabetes Mellitus to newly diagnosed patients and carers

3) Education of new and existing patients

4) Organise and oversee Annual/bi-annual diabetes monitoring programme to ensure safe, effective delivery of high quality evidenced based diabetic care, delivered face to face or by telephone that optimises the health and well-being of patients with diabetes.

5) Monitor and Assess achievement with Quality Framework regarding Diabetes

6) Management (Pharmacological and non-pharmacological) of patient’s glycaemic control, CHD risk and diabetic complications.

7) Referral to/liaison with other Health professionals if appropriate.

8) Clinically assess, manage and refer to specialist weight management programmes where necessary

9) Support patients to adopt health promotion strategies that promote a healthier lifestyle.

10) Oversee disease management within practice protocols.

11) Proactively identify and manage treatment plans for patients at risk of developing complications associated with diabetes and manage these patients.

12) Review medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence-based practice and national guidelines

Practice Nurse Duties

The post holder will be responsible for the day to day management of patients in the treatment room and will, when required:

1) Administer immunisation vaccinations and other injections to patients, as prescribed by the Doctors.

2) Carry out blood sugar and haemoglobin measurement, urine tests, ECGs and any other patient investigations as ordered by the Doctors.

3) Assist Doctors in performing gynaecological or other examination, as required.

4) Carry out cervical cytology.

5) Retain responsibility for patient continuing wound care, with referral to Doctors if necessary.

6) Provide a phlebotomy service for patients, as directed by the Doctors.

7) Carry out medication reviews as per practice protocols e.g. Hypertension, Pill checks, Hypothyroid

8) Operate a childhood vaccination clinic.

9) Maintain adequate stocks of drugs and supplies, in the treatment room and examination room.

10) Ensure adequate cleaning and sterilising of equipment.

11) Promote healthy living and lifestyles for all patients.

12) Assist in the induction and training of new staff along with trainee registrars and medical students.

13) Perform any other nursing tasks as required by the practice.

14) Identify vaccinations required for foreign travel and operate a travel clinic, giving relevant vaccinations/advice and discussing malaria prophylaxis.

15) Nurses holding the community nursing prescribing qualification will prescribe as appropriate from the formulary for dressings and wound management.

PROFESSIONAL ISSUES

Nurses will:

1) Keep accurate, contemporaneous electronic patient records for each patient in the appropriate manner.

2) Be aware of, and work in accordance with, all practice policies and protocols relevant to your role.

3) Identify their own training needs and actively participate in any training identified as being necessary by their manager or employer.

4) Participate in the training of new nursing staff.

5) Attend regular staff meetings, to maintain adequate communication.

6) Participate in staff development and performance review on which salary increments will be based.

7) Report to practice manager/GP Partner lead, any complaint, accidents, or any untoward incidents.

8) Work at all times in accordance with the NMC’s code of professional conduct and the scope of professional practice.

9) Act as a resource for information and advice to other nurses in the team.

PERSONAL ISSUES

Nurses will:

1) Be aware of and work in accordance with the Health and Safety at Work Act thereby working in a safe efficient manner.

2) Adhere to the uniform policy of the practice, always having a neat and tidy appearance.
