Great Bridge Partnerships for Health is seeking a part time HCA to join its friendly nursing team.
Our practice is across 4 separate sites, however this position will be based at our Izons Road site. And we would be happy to consider flexible working hours.
The role at the moment is 16 hours per week.
Main duties of the job
Candidates will ideally have experience in a primary care setting. However, applications may be considered if the relevant skills can be demonstrated. The role will include, long term conditions reviews, wound care management, adult immunisations, NHS Health checks, Ear irrigation and other duties to assist the nursing team. You should be able to demonstrate on your cv the ability to cope in a busy environment. You will need to have completed the NVQ level 3 in Health and Social Care. The Care Certificate would also be a desirable additional achievement.

Job responsibilities
- Operational Responsibilities
- Carry out new patient health
checks and NHS health checks.
- Reviewing patients with Long Term Conditions to
record routine measurements, offering disease management advice in accordance
with the practices clinical protocols.
- Carry out ECGs.
- BP monitoring
- To
undertake duties associated with 24 hour blood pressure monitoring to fitting
and downloading from patient.
- Phlebotomy
- Obtaining specimens e.g. urine,
and testing as instructed and completing appropriate laboratory forms and
contacting patients to follow up results.
- Processing and management of
laboratory samples requested by GPs/Nurses, ensure specimens are labelled and
bagged ready for collection with the necessary paperwork completed
- Weight
monitoring
- Routine Preventative Immunisations
- Carry out B12 vaccinations

Basic wound care/ulcer care and removal of sutures

Chaperoning and assisting patients where appropriate who are being examined by
another clinician
- Vaccine/cold chain storage, monitoring and recording

Surgical equipment and vaccine re-stocking and stock rotation
- Clearing and
re-stocking consulting rooms
- To monitor usage of sharps bins, ensuring that
rooms are adequately stocked and bins are transferred to storage for
collection.
- Ensure clinical waste collection runs smoothly, reporting any
problems to the Practice Manager
- Preparing and maintaining environments and
equipment before, during and after patient care interventions including
assisting GPs during the performance of minor operations
- Assisting in the
assessment and surveillance of patients health and well-being
- Undertaking
specific clinical activities for named patients that have been delegated and
taught specifically in relation to that individual
- Assist with childhood and
adolescent immunisations maintaining any Child Health and Practice records
including completing any necessary claims to the Health Authority.

raise awareness of health and well-being and how it can be promoted, provide
lifestyle advice to patients making any necessary referrals within the Practice
- Ordering and display of health promotional materials.
- Assist in and perform
routine tasks related to patient care as directed by senior nursing staff and
GPs.
- Maintain and clean equipment used by the nurses and GPs according to
the practice infection control policy.
- To order supplies as directed by the
nursing and management team.
- Undertake regular maintenance checks of water
temperatures and vaccine fridge temperatures.
- To co-ordinate the periodic
maintenance checks and calibration of medical equipment in accordance with the
practice policy.

- Undertake housekeeping duties, including general tidiness and
cleanliness of clinical rooms.
