Job summary

An exciting opportunity has arisen for a Quality Patient Safety Care Coordinator to join our friendly and dynamic team. The role is a part time role working Monday - Friday from 8am - 2pm ( 30 hours per week).

The purpose of the role is to develop and facilitate processes that support and maintain high quality care at the Nelson Medical Practice. Working with Senior Clinicians to improve patient care and ensuring compliance with CQC monitoring searches. To proactively communicate information between Doctors, patients and other healthcare profressionals.

Main duties of the job

The main duties of the role are;

  • To assist the doctors and other clinicians in the provision of high quality care.
  • To work proactively with Senior Clinicians to design and run processes that improve the safety of patient care, in particular safe prescribing and long term condition management.
  • Communicate with Patients and/or their carers to help with appointments and medication monitoring.
  • Liaise with the in-house contracts team to ensure regular recall is taking place in relation to those patients who require support in terms of managing long term conditions.
  • work with Clinicians to ensure medication monitoring and reviews are completed in a timely fashion.
  • Work with Patients and Clinicians in relation to Cancer safety netting template completion
  • Organise prescribing governance meetings, take minutes and prepare associated paperwork in relation to these meetings.

Job responsibilities

MAIN DUTIES AND RESPONSIBILITIES

1.

To assist the doctors and other clinicians in the provision of a high-quality patient care.

2.

To proactively work with senior clinicians to design and run processes that improve the safety of patient care in particular safe prescribing and long-term condition management

3.

Work with clinicians to develop EMIS searches and protocols to help facilitate effective prescribing

Communicate with patients (and or their carers) to facilitate appointments and medication monitoring testing.

4.

Ensure high level of achievement of CQC monitoring searches and additional practice specific safety protocol. Ensuring the practice achieves effective CQC monitoring targets.

5.

Develop additional monitoring searches according to practice priorities to improve patient care.

6.

Liaise with the in house Contracts Team to ensure regular recall is taking place in relation to those patients who are at the highest risk of complications from Long Term Conditions.

7.

Work with clinicians to ensure medication monitoring and medication reviews are completed in a timely fashion.

8.

Responsibility of cancer safety netting template and communication with patients and clinicians.

9.

Organizing & facilitating prescribing governance meetings. Taking of notes at team meetings & preparation of associated minutes.

10.

To work as a positive and active member within the Practice Team.

11.

You will be expected to take part in an annual appraisal at which your performance will be reviewed and your training needs discussed.

12.

Attend relevant meetings as required including staff meetings. You may be requested to attend other meetings at the request of the management team.

13.

To cover for the PCC team during team absences as directed by the PCC team leader
