Job summary
We are looking to recruit a Primary
Care Network Pharmacist.
You would be based at Ash Tree House,
in the town of Kirkham, Lancashire.
We are looking to strengthen our
already diverse team widening our skill set to bring us in line with the NHS
long term plan.
This post is looking for a GHPhC
Registered Pharmacist ideally with a GPhC Independent Prescriber qualification
and a minor ailments certification (or a willingness to undertake the required
training).
Experience in a community pharmacy
and or general practice would be desirable.
You will be competent in managing
repeat prescribing and advice relating to medication queries or enquiries.
You will be supported clinically by a
brilliant GP team and the wider Practice staff.

Main duties of the job
To
practice as an Independent prescriber.
To
carry out face to face and telephone consultations (as necessary) with patients
including medication queries, management of long-term conditions and the
management and review of patients with polypharmacy.
To
conduct structured medication reviews to all patients.
To
be responsible for the delivery of the prescribing element of the Healthy Fylde
Coast Contract.
To
work with the Practice team to support medicines optimisation.
Review
all safety alerts ensuring the required actions are taken.
To
reconcile medicines following patients discharged from hospital or clinic
appointments.
To
be responsible for monitoring the Eclipse Radar admissions avoidance system.
To
be actively involved and manage the relevant areas within QOF.
To
reauthorise medication for repeat prescribing within scope of practice.
Identify
patients suitable for repeat prescribing, liaising with the relevant clinicians
as required.

a progressive Training Practice housed in a beautiful listed building.
Ash Tree House provides a full range
of primary care services to a population of nearly 11,200 patients.
We are committed to giving our
patients the highest quality service by maintaining a mutual confidence between
Doctor and Patient. There are 5 GP Partners and 3 Salaried GP's working closely
with a dedicated Practice team which includes Practice Nurses, a Advanced Nurse
Practitioner, Counsellors and support staff.

Community staff including a Community Matron, District Nurses and a resident
Physiotherapist.

The
following are the core responsibilities of the Network Clinical Pharmacist. There
may be on occasion, a requirement to carry out other tasks; this will be
dependent upon factors such as workload and staffing levels:
1.
To practice as an independent prescriber working within
the scope of their clinical competencies to provide support and direction to
the practice team and the members of the allied healthcare team
2.
To directly engage with the patient population and carry
out face-to-face and telephone consultations (as necessary) with patients including
medication queries, management of long-term conditions and the management and
review of patients with polypharmacy
3.
To conduct structured medication reviews to all patients including
those in local residential or care settings in conjunction with local
services such as the Care Home Team
4.
To be responsible for the delivery of prescribing element

practice and the wider healthcare team
18.
To communicate effectively with the local community
pharmacies to both foster a good working relationship and to communicate
practice initiatives

addition to the primary responsibilities, the clinical pharmacist may be
requested to:

Partake in audits as directed by the Partners
b.
Provide education on medicines for the clinical team
including prescribing guidance to trainees and medical students
c.
Review the latest guidance ensuring the practice conforms
to NICE, CQC etc.
d.
Act as the practice point of contact for all medicines
related matters, establishing positive working relationships
