Job summary

The Care Coordinator / Healthcare Assistant plays a key role in supporting patients to receive high quality, personalised, and coordinated care. Working closely with GPs, nurses, and the wider practice team, the postholder will manage a defined caseload of patients - particularly those with long term conditions, frailty, or complex needs - while also contributing to the safe and effective delivery of clinical care as a Healthcare Assistant.

This role supports proactive, preventative care, improves continuity, and helps patients to better understand and manage their health and wellbeing.

Main duties of the job

  • Proactively identify and work with a cohort of patients to support their personalised care requirements
  • Provide coordination and navigation support using digital tools to help patients access appropriate services
  • Develop and maintain personalised care and support plans based on an individuals needs and what matters to them
  • Promote preventative heath care and continuity of care
  • Assist the clinical team in the provision and delivery of safe and effective care to the practice population,working within agreed guidelines, protocols, and scope of practice

Job responsibilities

Care Coordination Duties

  • Proactively identify and work with a cohort of patients to understand and support their personalised care needs, particularly those with long term conditions or at risk of frailty.
  • Coordinate and navigate care for patients and their carers across health, social care, and voluntary services, supporting referrals and ensuring timely access to appropriate services.
  • Use IT systems and digital tools to proactively coordinate care, including managing and tracking carers involvement, learning disability reviews, dementia and frailty support, and neurodiversity referrals. This includes maintaining accurate spreadsheets, using clinical system searches, registers and reports, and ensuring information is kept up to date to support safe, timely and joined up care.
  • Support patients and carers to prepare for, and follow up, clinical conversations with primary care professionals, enabling them to be actively involved in managing their care.
  • Facilitate effective communication between patients, the primary healthcare team, secondary care, and other relevant agencies.
  • Develop, implement, and regularly review personalised care and support plans in collaboration with patients, carers, and clinicians.
  • Promote preventative healthcare, self-management, and continuity of care in line with practice and PCN priorities.

Healthcare Assistant (HCA) Duties

  • Assist the clinical team in the provision and delivery of safe and effective care to the practice population, working within agreed guidelines, protocols, and scope of practice.
  • Undertake phlebotomy procedures, including taking, labelling, storing, and dispatching specimens accurately.
  • Perform blood pressure monitoring and record results appropriately.
  • Support longterm condition reviews in line with practice protocols.
  • Carry out ECGs and ensure accurate recording and reporting.
  • Maintain accurate, timely clinical records on the practice clinical system.
  • Ensure infection prevention and control standards are maintained at all times.

General Responsibilities

  • Work as part of the wider practice team to support high standards of patient care and service delivery.
  • Maintain patient confidentiality and comply with GDPR, information governance, and practice policies at all times.
  • Participate in training, supervision, and appraisal, and maintain mandatory competencies.
  • Contribute to quality improvement, audits, and service development where appropriate.
  • Promote equality, diversity, and dignity in all aspects of care.
