Job summary

Jubilee Park Medical Partnership is looking to recruit a Care Coordinator to join our newly merged practice on a permanent basis.

We are a friendly 2 sited practice with strong clinical and non clinical teams. Our teams are dedicated and hardworking and put the patient at the centre of all we do.

The role is a part-time position offering 24-30 hours on a rota basis and will involve cover of our 7am opening as well as occasional cover of our evening sessions up to 8pm. Your time will be spent working across both of our sites.

Main duties of the job

The successful candidate will have excellent inter-personal, administrative and organisational skills.

Applicants are required to have excellent computer and communication skills, both face to face and telephone.

Applicants need to be smart, professional, friendly, reliable and punctual. You will need to work well under pressure, be able to multitask, pay attention to detail and have excellent customer care skills.

Flexible working is essential to provide additional hours when required to cover staff holidays and absence.

About us

This is an exciting opportunity to join the newly merged Jubilee Park Medical Partnership (Park House Medical Centre and Jubilee Practice). Our merger is effective on 1st April 2020.

Both practices are well established and CQC rated GOOD practices. We have been working closely together for the past 2 years and are now about to take the next step in this exciting journey.

Job description

Job responsibilities

The following are the core responsibilities of the Care Coordinator. There may be on occasion, a requirement to carry out other tasks; this will be dependent upon factors such as workload and staffing levels:

a. Maintaining and monitoring the practice appointment system

b. Process personal, telephone and e-requests for appointments

c. Answer incoming phone calls, transferring calls or dealing with the callers request appropriately

d. Process and effectively signpost patients to the appropriate healthcare professional depending on the presenting condition

e. Initiate contact with and respond to requests from patients, team members and external agencies

f. Read code data on SystmOne

g. Photocopy documentation as required

h. Data entry of new and temporary registrations and relevant patient information as required

i. Input data into the patients healthcare records as necessary

j. Direct requests for information i.e. SAR, insurance / solicitors letters and DVLA forms to the administrative team

k. Manage all queries as necessary in an efficient manner

l. Raise queries via tasks to appropriate member of staff and deal with tasks in the reception task list in an appropriate order

m. Action tasks set by clinicians and other members of the practice team via the clinical system and by email

n. Sort and route external mail to include scanning attachments to the electronic clinical record following practice policy.

o. Assist the practice in the call/recall of patients

p. Complete opening and closing procedures in accordance with policy

a. Support the management of repeat prescriptions, ensuring they are processed accurately and efficiently

b. Distribute completed prescriptions

c. Distribute dispensed medication ensuring that patients correctly complete the exemption boxes and give correct payment

q. Action incoming emails

r. Carry out system searches as requested

s. Maintain a clean, tidy, effective working area at all times

t. Monitor and maintain the reception area and notice boards

u. Support administrative staff, providing cover during staff absences

d. Ordering and monitoring of stationery supplies

e. Attend organised training events and updates for non-clinical staff
