Job summary
We are looking for a caring and motivated Clinical Pharmacist to join our friendly and supportive practice team for a fixed period of 2 months. You will work closely with our GPs and experienced pharmacy team to help ensure our patients receive safe, effective and personalised care, while taking a proactive role in medicines optimisation and patient safety.
The pharmacy team comprises a lead pharmacist, clinical pharmacist and pharmacy technician .
We are seeking support asap for this team for a two month period of up to 15 hours per week. There is some flexibility in when and how this is worked.
We will be reviewing submitted applications as they arrive and will interview and close the application as soon as a suitable candiate is found.
Main duties of the job
You will support patients to get the very best from their medicines by reviewing and optimising prescribing in line with agreed protocols and best practice guidance.

Prescription management:
- Amending or updating repeat template
- Acute medication requests
- Alternative medication requests due to shortage or patient request
- Prescription re-authorisation
- Liaising with community pharmacies,social care, secondarycareor specialists e.g. advice and guidance requests
- Implementing formulary changes requestedby practice.
- Synchronising medication
- Reissuing repeat dispensing batch prescriptions
Medication safety activities:
- Ensuring medication monitoring is up to date and using the recall system toinform patients when reviews are due.
- Proactively chasing non-responders with measures in place to limit unsafe prescribing e.g. reducing quantities
Document management
- Clinic and discharge letter review and medicines reconciliation
- Contact with patients to discuss changes and organise review if appropriate
Medication reviews:
- Notes based medication review supported by medication specific questionnaires
- Support the practice to deliver QOF targets
- Post discharge medication reviewswhere appropriate
- Titration of medication or de-escalation of treatment e.g. heart failure, blood pressure management, dependence forming medicines
- Blood test results requiring patient contact e.g. raised lipids/QRISK scores

Support and action digital software reports specific to prescribing and drug monitoring
To review medicationsand set up repeat templatesfor newly registered patients
