We are looking for a reliable, enthusiastic and motivated Practice Nurse to work with us. A nurse who can provide excellent care and has a true dedication for improving health outcomes for patients.

The successful candidate will be a registered nurse, ideally with experience of working in General Practice.

The work is interesting, often challenging but hugely rewarding. The team is mutually supportive, with excellent experienced clinical, management and administrative support onsite.

About us

Sothall Medical Centre is a very friendly and supportive place to work. We have a stable patient base of around 10,000 patients. Our Nursing team consists of 2 ANP's, one practice Nurse (+ the successful candidate) one Nurse associate, one TNA, two HCA's and we are recruiting another HCA soon.

Main duties of the job

To work as a member of the Practice Nursing Team and be responsible for acting within their professional boundaries to provide care for the presenting patient including screening, clinical assessment, diagnosis, treatment and follow up of care; demonstrating safe clinical decision making and expert patient care

To be involved in all areas of Chronic Disease Management (Asthma, Diabetes, COPD)

The Practice Nurse will work collaboratively with the clinical team which will include GPs and other health care professionals.

2. Main Duties and Responsibilities

Key Result Areas

2.1 Clinical Care

- Assist in and perform routine tasks relating to patient care

- Undertake and promote cervical cytology

- Manage newly diagnosed patients with Asthma, COPD & Diabetes

- Undertake Chronic disease management and prevention Asthma, COPD, Diabetes, Hypertension, Hypothyroidism, IHD, Stroke & LD

- Perform wound care using aseptic technique and general wound care

- Undertake opportunistic screening of BP checks, height, weight, BMI, urinalysis and smoking status

- Administer injections under an individual prescription or patient group direction

- Offer a holistic approach to travel health including comprehensive advice for patients prior to travel

- Undertake ear syringing

- Support patients and colleagues in adopting sound infection control measures

About us

Sothall Medical Centre is based in the South East of Sheffield close to Rother Valley Country Park and Crystal Peaks retail. We are a relatively large practice with around 10,000 patients. We have a great supportive and friendly outlook and encourage staff to develop their skills and abilities. We have a very supportive and friendly Nursing team who are a vital part of the Practice and regularly praised by our patients as well as our practice staff. The practice has a wide range of other clinical staff, we have 3 Partners, 5 salaried GP's (2 on MAT leave at the moment). A Physician Associate, A clinical Pharmacist, a Pharmacy technician and an apprentice Pharmacy technician. The practice is very stable and the future of our practice is looking promising. We have a very close Network with other local practices and have network specialist staff supporting our patients in our practice (e.g. Physiotherapists)
