Job summary
We are a GP Practice based in the Barnton area of Northwich (Cheshire) looking to appoint a further Care Coordinator to our amazing team.
As a Practice, we want to develop our capacity to support our patients and look at exciting future projects, therefore while the job title is 'Care Coordinator', the role is a lot more.
The main functions of the role are to:
Work alongside the Practice Management Team on projects relating to PCN and practice targets.
Work with patients and the Practice on Patient communications such as (but not limited to): information boards, practice website, social media, leaflet design and information.
Undertake the role of the Care Coordinator within Practice (1:1 with patients), alongside cover for general administration duties as required by the Practice.
All of our administrators work across varying roles, including reception, prescribing admin, secretarial, documents and coding. We want our staff to develop in an area they are excited and passionate about, as well as being able to support each other, therefore we offer all staff the opportunity to experience all parts of the admin role.

Main duties of the job
- To manage projects, on direction of the Management Team, to support the Practice and PCN achievement of IIF and QOF under the instruction of the Senior Care Coordinator and Practice Management Team.
- To deputise for the Senior Care Coordinator when needed in areas such as: Palliative Care meetings, Care Home reviews, Practice newsletters
- To be a point of contact within Practice for patient queries and issues, either directly as part of the Care Coordinator role, or through supporting the Practice Team.
- To develop with the Practice Management Team and Patients a Practice communications strategy developing original materials and information on various media such as websites, social media, leaflets and notice boards.
This is a diverse role, incorporating 1:1 patient support improving patient experience, and working with the Practice team to develop better communication between the Practice and Patients.
You will have support in the role but will be expected to work autonomously, providing ideas and suggestions, as well as working with all members of the team.
