- Job
Summary
The post holder
must be registered nurse with post graduate qualifications, relevant to the
position. They must act within their professional boundaries, providing care
for the presenting patient, working within their competencies. This will
involve telephone triage/consultation, history taking, clinical assessment,
diagnosis, treatment and evaluation of their care. They will demonstrate safe, clinical
decision-making and expert care for patients within the general practice. They will work collaboratively with the
multi-disciplinary general practice team to meet the needs of patients, abiding
by local policy and procedures.
The post holder
will work as part of the urgent care team within the Acute Care Hub, as well as
providing flexibility to carry out necessary home visits, allocated on a daily
basis, under the guidance and support of the GP.

Key Result Areas
Clinical Care
- Act as an expert practitioner,
demonstrating clinical competence and a sound knowledge base.
- Make direct referrals to
primary, secondary and social services within locally agreed pathways,
guidance and protocols.
- Provide clinical advice
to Reception staff where required.
- Ensure that patients
receive high quality clinical care, delivered in a timely manner.
- Ensure complete and
accurate documentation of each and every patient contact.
- Work as an independent
practitioner and as part of the multidisciplinary and multi-agency teams,
in order to ensure patients needs are met.
- Ensure that professional
standards are maintained and within the guidance of the Department of
Health and the Nursing and Midwifery Council.
Professional
- Adhere to own
professional body requirements and always work within the scope of
professional practice.
- Ensure that professional
practice adheres to organisational policies, procedures and guidelines.
- Maintain a professional
manner and act as a role model and mentor for junior staff; including
trainees and student nurses.
- Maintain confidentiality
with regard to information pertaining to patients and staff.
- Ensure that the service
interfaces with all other departments in a professional and productive
manner, providing an effective service to partner organisations and other
service providers.
Communication
Communicate effectively with patients and
families regarding complex information about their care
Provide advice and information to patients,
carers and their families where appropriate
Communicate effectively and work collaboratively
with medical, nursing and allied health professionals, to ensure delivery of a
co-ordinated service.
Ensure accurate documentation and record
keeping, in line with local policies and own professional body guidelines.
Promote and demonstrate effective
communication networks within the organisation, with partner organisations and
other service providers
Clinical
Governance
Work with the Clinical leads to investigate
adverse clinical incidents and complaints.
Adhere to St Austell Health Care, procedures
and guidelines
Service Improvement
Contribute to quality improvement within the
service through the identification and initiation of changes, which lead to
better evidence-based practice and improved standards of care
Participate in clinical audit, identifying areas
of best practice and areas for improvement.
Contribute to the setting and monitoring of
standards of care.
Help develop the services available to the
urgent care population to reflect identified needs
Act as a mentor /supervisor of staff as
appropriate
Educational/Personal
Development
Contribute to the
development and maintenance of an excellent learning environment
Work with a
Director /supervisor/mentor to support clinical practice and development of
knowledge/skills.
Adhere to
professional registration requirements.
Participate
in defining your own developmental plan to maintain and develop own clinical
skills and ensure own professional development
Undertake and
complete all statutory and mandatory training

Confidentiality
All staff

Information Governance
All staff must keep up-to-date with
the requirements of Information Governance and must follow organisational policies and
procedures to ensure that the organisations information is dealt with legally,
securely, efficiently and effectively. Staff must appropriately manage all the
Information they handle during the course of their employment with St Austell
Health Care, making the information available for sharing in a controlled
manner, subject to statutory requirements.
Data Protection
St Austell Health Care is registered as a data controller under the
Data Protection Act 1998. All personal information we hold, obtain, record, use
and share as an organisation is governed by this Act. All employees have a legal responsibility for
all personal information handled
and must not, at any time, use the personal data in a way incompatible with the guidelines stipulated in this act. Any doubts regarding what should or should

immediate line manager.
Records Management
All employees have a legal
responsibility for all records worked with e.g. patient records, financial
records, personal, administrative etc., that are gathered or used as part of
working within the organisation. The records may be held in a variety of
formats such as paper, electronic, microfiche, audio and video tapes etc. Any doubts regarding the correct management
of records must be discussed with immediate line manager.
