Job summary

The post holder must be registered nurse with post graduate qualifications, relevant to the position. They must act within their professional boundaries, providing care for the presenting patient, working within their competencies. This will involve telephone triage/consultation, history taking, clinical assessment, diagnosis, treatment and evaluation of their care. They will demonstrate safe, clinical decision-making and expert care for patients within the general practice. They will work collaboratively with the multi-disciplinary general practice team to meet the needs of patients, abiding by local policy and procedures.

The post holder will work as part of the urgent care team within the Acute Care Hub, as well as providing flexibility to carry out necessary home visits, allocated on a daily basis, under the guidance and support of the GP.
