Job summary

We are looking for a GP to cover 6 sessions a week days negotiable .

We are a practice serving a population of 12000 patients with a practice in the centre of Salisbury and a 2nd practice in Larkhill .

Our Larkhill branch is a dispensing practice .

We are proud to offer triage based care for our patients.

Our team consists of 7 GPs , Full Nursing Team and Advanced Nurse Practitioners.

Main duties of the job

  • Make professionally autonomous decisions for which he/she is accountable

  • Making use of skills in history taking, physical examination, problem-solving and clinical decision-making, to establish a diagnosis and management plan
  • To maintain accurate and contemporaneous records, utilising computer systems where appropriate and consider the Caldicott Principles in relation to all data handling.
  • Instigate necessary invasive and non-invasive diagnostic tests or investigations and interpret findings/reports
  • Provide safe, evidence-based, cost-effective, individualised patient care
  • Offer a holistic service to patients and their families
  • Promote health and well-being through the use of health promotion, health education, screening and therapeutic communication skills
  • Refer patients directly to other services/agencies as appropriate
  • Support the other clinicians within the Practice in developing and delivering specialist primary care services. To work within the practice guidelines, policies and protocols.
  • Contribute to the practice achieving its quality targets to sustain the high standards of patient care and service delivery

About us

We are a team with a strong sense of working together. The admin and reception team play a vital role in supporting the clinical team on a daily basis.

The ANPs look after the same day care for all patients through triage system ensuring all patients requesting a call or appointment are contacted the same day of request.

We work in a primary care setting our excellent Clinical team including 7 GPs, 5 ANPs, 2 Pharmacists, 3 Nurses, 4 HCAs, 1 Paramedic and 2 Mental Health Support Workers. We ensure clinical safety and facilitate senior-level support and guidance. We are located in Salisbury and have branch site in Larkhill. The Partnership team is a true Multidisciplinary team consisting of a GP partner, Advanced Nurse Practitioner Partner and a Pharmacist Partner and the practice is very keen on the development of individuals and roles.

Job responsibilities

Provide a full range of medical services as defined in the core GMS contract and additional enhanced services where appropriate

Assumes a clinical workload dependant on the individual general practice population in collaboration with MDT colleagues or as determined by a specific patient group when involved in specific clinics.

Independently and autonomously assess, diagnose, plan, implement and evaluate treatment/interventions and care for patients presenting with an undifferentiated and undiagnosed problems with attention to physical, emotional, social, cultural and mental health issues

Assess, diagnosis, plan, implement and evaluate interventions/treatments for patients with complex needs including effective care plan management

Proactively identify, diagnose and manage treatment plans for patients at risk of developing a long-term condition (as appropriate)

Diagnose and manage both acute and chronic conditions, integrating both drug- and non-drug-based treatment methods into a management plan

Prescribe and review medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence-based practice and national and practice protocols, and within scope of practice

Accept referrals from other disciplines unsupervised

Refer, admit and discharge patients to and from caseloads including external agencies such as secondary care and social services

Work with patients in order to support compliance with and adherence to prescribed treatments

Provide information and advice on prescribed or over-the-counter medication on medication regimens, side-effects and interactions

Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care

Support patients to adopt health promotion strategies that promote healthy lifestyles, and apply principles of self-care

Support and manage health needs of women presenting for family planning, cervical cytology or sexual health consultation
