Job summary

This is an exciting brand new role to support the Clinical Directors, Business Manager and Practice Managers to develop the Forest of Dean Primary Care Network and effectively deliver key projects including Directed Enhanced Services and supporting Quality and Outcomes Frameworks.

Main duties of the job

You will need excellent communication skills and experience of developing working relationships as well as an understanding and experience of working in the NHS with detailed knowledge of the roles of NHS organisations and of the primary care sector.

You should also have an understanding of national policy direction for primary, community and secondary care as evidenced in the NHS Long Term Plan, together with good practical and conceptual knowledge of healthcare improvement methods and practices.

You also need experience of leading and delivering projects within tight timescales, will be self-motivated, and have the ability to use initiative to decide actions and make recommendations.

Job responsibilities

Work with and support the PCN Clinical Directors to build and develop the Forest of Dean Primary Care

Undertake specific project work as required by the PCN.

Facilitate effective communication and engagement with and between member practices and to represent the PCN in wider locality and system forums.

Effectively deliver key projects including the provision of information and analysis. This will be achieved through the effective use of information, data, knowledge and technology systems to inform and support PCN agreed initiatives and meet the needs of the local population.

Main duties and responsibilities

Manage the delivery of the three main specifications set out in the 20/21 PCN DES (and future DESs)

Support the Clinical Directors, Business Manager and Practice Managers in the implementation and progression of PCN projects and contracts

Lead on specific projects agreed by Clinical Directors and ensure they are delivered in line with project timescales.

Report to Clinical Directors on projects which are not achieving key milestones

Support the setting of strategic goals for the PCN and preparing action plans.

Liaise and work with the CCG, practices, voluntary and other organisations to implement patient services

Promote a culture which encourages teamwork, sharing of best practice and continuous improvement

Help achieve objectives as measured in the PCN dashboard, with a view to obtaining maximum investment and impact funding

Prepare development plans as required

Encourage supporting staff to collaborate through sharing information and intelligence across different activities.

Support and help to integrate the ARRS workforce to ensure maximum benefit, reduce duplication and improve efficiency and effectiveness.

Support the PCN with the delivery of improved population health outcomes

Initiate and develop a portfolio of projects related to the promotion, recruitment, workforce planning and the professional support of the ARRS roles.

Attend meetings on behalf of the PCN

Raise awareness of health promotion in practices, implementing, co-ordinating and supporting a variety of projects.

To support the Clinical Directors in the delivery of the DES specifications and support Quality and Outcome Frameworks.
