Job summary
East Barnwell Health Centre, a well-established training practice in Cambridge, seeks a highly-motivated, experienced Practice Nurse to join our dedicated team
Main duties of the job
To be fully proficient in Chronic disease management, vaccinations and immunisations, cervical screening, contraceptive reviews, wound care including compression, as well as other general treatment room duties.
To provide support to junior members of the nursing team, including healthcare assistants and GPAs and direct their work to ensure that the Practice provides high quality care.
To oversee infection control standards and cold chain compliancy.
To work within NMC Code of Conduct and to agreed Practice standards and protocols
To oversee recall systems for chronic diseases, cervical screening, immunisations, and other key areas as relevant to Practice Nursing
To deliver effective management of patients with Chronic Diseases as detailed by the Quality and Outcomes Framework, local quality schemes and national quality schemes
To Support the implementation of evidence-based guidelines in conjunction with the wider team
To encourage the development of integrated services within Primary Care to include integrated practice-based teams and multidisciplinary working
To ensure that all nurse-related Care Quality Commission evidence table indicators are compliant, demonstrating evidence, review, and audit cycle

We are an established, well organised, supportive practice in Cambridge serving 7,800 patients. We have four partners, a team of salaried doctors, retainer doctors, nurses and nursing assistants, physiotherapists, clinical pharmacists and social prescribers. We have a strong interest in teaching and training medical students, nursing students and GP trainees. We play an active role in Cambridge City PCN.

Job responsibilities
PURPOSE OF ROLE
To provide and maintain high standard of nursing care, in conjunction with nursing team colleagues, to the practice population.
To focus in the delivery of evidence based practice for patients presenting with long term conditions and the provision of preventative health care to the practice population.
To provide nursing care to practice patients to include primary care treatment, preventative care, screening and education.
CLINICAL RESPONSIBILITIES
Provide Chronic disease management for Diabetes, COPD, Asthma, and CHD/CVD
Perform cervical screening and promote breast awareness
Administer adult and paediatric injections
Provide adult and child immunisations, maximising achievement of targets
Ensure safe storage, rotation, and disposal of vaccines and drugs
Undertake venepuncture and electrocardiography
Undertake wound care/ management / dressing techniques
Undertake leg ulcer management, dopplers and compression bandaging
Provide advice on lifestyle issues, assisting and supporting patients in changing their unhealthy behaviours and refer appropriately
Participate in the smoking cessation service
Deliver travel health care, including vaccinations and advice for patients
Recognise the signs of child abuse, family violence, vulnerable adults, substance abuse, addictive behaviours, suicidal tendencies, and be aware of local, referral procedures
Work with safeguarding teams where relevant
Ensure that current evidence-based approaches to patient care are standard practice
Provide a comprehensive family planning service
Audit outcomes of care against standards and initiate change as necessary ensuring follow-up audit cycle

ADMINISTRATIVE AND PROFESSIONAL RESPONSIBILITIES
Participate in the administrative and professional responsibilities of the practice team.
Ensure accurate notes of all consultations and treatments are recorded contemporaneously in the patient computerised record and where appropriate, in their written notes using System one.
Ensure accurate completion of all necessary documentation associated with patient care and registration with the practice.
Ensure collection and maintenance of statistical information required for regular and ad hoc reports.
TREATMENT ROOM SUPPLIES AND EQUIPMENT
Clean relevant equipment and maintain supplies of appropriate consumables in accordance with the Practice Infection Control Policy.
COMMUNICATION
Maintain effective communication and co-ordination with other members of the practice nursing team.
Maintain effective liaison with other agencies and staff concerned with patient care and all other disciplines within the practice, with appropriate regard to confidentiality.
PROFESSIONAL DEVELOPMENT
Attend and participate in meetings with other members of the practice team.
Maintain continued education by attendance at courses and study days as deemed useful. and necessary for professional development or the work being undertaken by the practice.
Undertake an annual review in according with the Practice Performance Management System.
Maintain any status/registrations required to undertake the duties of the role (e.g. revalidation)
Ensure all mandatory training is kept up to date.
INFECTION CONTROL
Awareness of national standards of infection control and cleanliness and regulatory/contractual/professional requirements, and good practice guidelines.
Responsible for the correct and safe management of the specimens process. including collection, labelling, handling, use of correct and clean containers, storage and transport arrangements.
Management and maintenance of Personal Protective Equipment (PPE) for the practice including provision, ordering, availability and on-going correct usage by staff.
Monitoring practice facilities and equipment in relation to infection control, ensuring that provision of hand cleansing facilities, wipes are sufficient to ensure a good clinical working environment. Lack of facilities to be escalated as appropriate.
Safe management of sharps procedures including training, use, storage and disposal.
Use of appropriate infection control procedures, maintaining work areas in a tidy, clean and sterile and safe way, free from hazards. Initiation of remedial/corrective action where needed or escalation to responsible management.
Undertaking periodic IPC training.
Responsible for Waste management including collection, handling, segregation, container management, storage and collection.
