An exciting opportunity has arisen to join Barnt Green Surgery as a Pharmacy Technician to support our growing pharmacy team.
The Clinician Pharmacist along with 2 Prescribing clerks is part of a dedicated wider team comprising of of 3 Partners, 2 Salaried GPs, a Physicians Associate, a nursing team, reception and and admin team. The successful candidate will work alongside our existing team to deliver safe, effective and cost-effective use of medicines by applying their pharmaceutical, clinical & technical skills. The role will provide a brilliant opportunity for the right candidate to develop their knowledge and skills.
Main duties of the job
Within the practice, the post holder will
work as part of a multi-disciplinary team in a patient-facing role.
To provide specialist pharmaceutical
technical services, professional advice and direct support to Practice GPs,
Nurses, Pharmacist, HCAs, physicians associate and Reception/Administrative
Staff.
To liaise with patients, clinical and
non-clinical staff, practice, hospital and community pharmacists and patient's
carers to improve the pharmaceutical care of patients.
To develop the role of the Practice Pharmacy
Technician to maximise support to all Clinical and non-clinical practice staff
to improve patient care in relation to all aspects of medicines and their
impact.
To provide professional specialist medicines
advice to GPs, Nurse Practitioners, Practice Nurses, Community Pharmacists,
Community Nurses and other health professionals to promote the safe, effective,
high-quality and cost-effective use of medicines in all prescribing decisions
for the benefit of the practice's patients.
Ensure that the practice integrates with community and
hospital pharmacy to help utilise skill mix, improve patient outcomes, ensure
better access to healthcare, and help manage workload.

KEY DUTIES AND RESPONSIBILITIES OF THE ROLE:
To respond to
medication queries from patients and staff in a professional manner, referring
to the appropriate clinician in accordance with practice protocols.
To support the
Pharmacist in providing structured medication reviews, developing further
Standard Operating Procedures, patient information leaflets and information
about medicine awareness.
Carry out medicines
optimisation tasks including effective medicine administration (e.g. checking
inhaler technique), supporting medication reviews and medicines reconciliation.
Where required, utilise consultation skills to work in partnership with
patients to ensure they use their medicines effectively and to reduce medicine
wastage.
Support medicines
reconciliation for new care home patients and synchronising medicines for
patient transfers between care settings and linking with local community
pharmacists.
To actively manage
discharge summaries, and clinic letters (Docman), actioning and updating
patients records, reconciling medications, and liaising with patients when
necessary.
Provide specialist
expertise, where competent, to address both the public health and social care
needs of patients, including lifestyle advice, service information, and help in
tackling local health inequalities.
To liaise where
necessary with community pharmacists and other pharmacists in relation to medicine
optimisation and medication shortages.
To participate in
medication and other clinical audits, including high risk drug monitoring.
To take a proactive
approach to the safe handling of prescriptions, including assessment of risk
and making recommendations for improvement.
To support the
achievement of the practices prescribing Quality and Outcomes Framework
targets and other ICS or Enhanced Contracts (e.g. DES contracts, IIF, QOF, CEIF
targets).
To assist in the
control of medicines stocks within the practice, ensuring compliance with
medicines legislation.
To work alongside
administrative staff involved in the preparation of repeat prescriptions daily
to ensure timely, safe, high-quality effective repeat prescribing.
Manage shared care
protocols and liaise with Clinical Pharmacists for more complex patients.
To help plan, develop
and support the introduction of new working processes within the practice to
optimise the quality of prescribing and to review prescribing within the
practice to ensure cost-effective prescribing occurs in line with CCG medicines
management team.
To action MHRA alerts
and drug safety alerts.
