Job summary
An
exciting opportunity has arisen for a Practice Pharmacist to join our dynamic
and forward-thinking team at Brookside Group Practice. We are looking for 15-20 hours per week.
The
Practice Pharmacist will be responsible for support to the practice with
medication reviews, queries and the delivery of the repeat prescription
service. Also, part of the group that implements effective medicine management
within the practice, identifying areas for improvement, and initiating and
managing change.
The successful candidate will ideally have a specialism in diabetes and be able to conduct diabetic reviews or be willing to work towards this.
Main duties of the job
To
provide a medication review and query services to patients via the telephone or
face to face clinics as appropriate in the practice.
Ensure
data is recorded so that patient care is enhanced (eg coding, reminders) and so
that it can be retrieved in a meaningful way to help manage patient care and
help with data analysis.
It will be necessary to attend and contribute to
various practice meetings as requested.

Winnersh and Wilderness Road.
