Main duties of the job

As an independent and proactive practitioner, you'll manage a diverse clinical case load within a primary care setting.

To ensure we maintain the highest standards, you'll meet all NMC requirements and work diligently within the NMC Code of Conduct, following agreed practice standards and protocols.

We believe in striving for excellence in patient care, and that's where you come in. Chronic Disease management will be a vital part of your role. Familiarity with The Quality and Outcomes Framework, LES/DES, is an element of this dynamic position.

2. Take the initiative in creating and maintaining health screening and promotion clinics for specific areas of our practice's diverse population, always on the lookout for opportunities to enhance systems or refer patients when needed.

3. Play a vital role in the review of chronic diseases like diabetes, COPD, asthma, or CHD. When appropriately trained, collaborate with other clinicians to ensure comprehensive medical reviews.

4. Administer adult and child immunizations with skill and confidence, adhering to national and local standards.

5. Take charge of various essential procedures, such as venepuncture, glucose tolerance tests, BP management, and more.

6. Recognize and prioritize the importance of identifying, documenting, and communicating signs and symptoms related to child and adult abuse, ensuring the safety and well-being of vulnerable patients.

7. Follow and adhere to all practice policies and procedural guidelines, ranging from infection control to patient group directions, making every effort to enhance patient care.

8. Foster seamless communication, build strong relationships, and work effectively within our collaborative healthcare team, including referrals to specialized practitioners when needed.
