Primary Care Clinical Pharmacist

COCO PCN is an innovative, dynamic PCN combining 3 local GP Practices covering Addlestone, Chertsey and Ottershaw. We are continually seeking new ways to improve local delivery of healthcare services for our population to enable residents to live the healthiest lives possible.

An exciting opportunity has arisen for a Clinical pharmacist to join our multi-disciplinary team within CoCO. It is a requirement of the role to be registered with the General Pharmaceutical Council along with Membership of the Royal Pharmaceutical Society. Primary care experience would be an advantage but is not essential. Flexibility, enthusiasm and the ability to prioritise tasks effectively are essential skills.

Job Description

Title: Clinical Pharmacist

Responsible to: CoCO Primary Care Network

Main Purpose of the Post:-

To be responsible for, leading on, and supporting implementation of an effective medicine management system within the PCN, including cost-effective prescribing, working as an independent prescriber and providing advice and support to multi-disciplinary colleagues. In additional the post-holder will also work with the strategic objectives of CoCO PCN specifically in relation to supporting medical review work in our care homes.

The role will also include training and development of practice based prescribing clerks and administrators to support improved co-ordination and effective pharmacy administration within general practice, especially with relation to repeat prescribing systems and processes. You will also be expected to:-

Develop and facilitate a good working relationship with community pharmacists and other local providers of healthcare

Plan and organise the post holders own workload, including audit and project work, and training sessions for members of the practice team, community pharmacy team, community nurse team, patients, carers, etc.

Record and maintain a database of information relating to the work done in the PCN. This includes any work relating to audits undertaken by the post holder and reference notes relating to clinical/technical information.

Maintain registration as a pharmacist and comply with appropriate professional codes

As appropriate to the post, maintain and develop professional competence and expertise, keep up to date with medical/therapeutic evidence and opinion, and local and national service, legislation and policy developments, agree objectives and a personal development plan and participate in the appraisal process

Attend local, regional and national meetings of relevance

Undertake any other duties commensurate with the post holders grade as agreed with the post holders line manager

Qualifications

Essential

• Masters degree in pharmacy (MPharm)
• Mandatory registration with General Pharmaceutical Council
• Membership of the Royal Pharmaceutical Society

Desirable

• A member of or working towards Faculty membership of the Royal Pharmaceutical Society
• Clinical diploma
• Independent prescriber

Experience

Essential

• Minimum of 2 years post-qualification experience.
• An appreciation of the nature of GPs and general practices .
• An appreciation of the nature of primary care prescribing, concepts of rational prescribing and strategies for improving prescribing .
• Excellent interpersonal, influencing and negotiating skills.
• Excellent written and verbal communication skills.
• Able to obtain and analyse complex technical information
• Recognises priorities when problem solving and identifies deviations from the normal pattern and is able to refer to seniors or GPs when appropriate
• Able to work under pressure and to meet deadlines
• Produce timely and informative reports
• Demonstrate the ability to communicate complex and sensitive information in an understandable form to a variety of audiences (e.g. patients)

Desirable

• In depth therapeutic and clinical knowledge and understanding of the principles of evidence-based healthcare.
• Is able to plan, manage, monitor, advise and review general medicine optimisation issues in core areas for long term conditions.
• Gain acceptance for recommendations and influence / motivate / persuade the audience to comply with the recommendations / agreed course of action where there may be significant barriers
