Main duties of the job

  2. Clinical Excellence: Implementing evidence-based practices and protocols to ensure the delivery of safe and effective healthcare interventions.

  3. Collaborative Approach: Working closely with multidisciplinary teams to coordinate patient care plans and optimise health outcomes.

Job responsibilities

HOURS: As per contract

Job summary:

To provide high quality, evidence based nursing care to meet the healthcare needs of the registered Practice population.

To act as Infection Control Lead for the Practice.

To maintain high standards of nursing care and work within the scope of the NMC code of professional conduct.

Key Responsibilities:

Clinical:

To provide a range of nursing services including screening, treatment and advice within the professional competence of the post holder

To utilise appropriate assessment frameworks to establish the health needs of patients / carers

To provide health education and work with the patient and carer where appropriate, alongside other healthcare professionals to effectively deliver appropriate packages of care

To provide assessment, implementation and evaluation of clinical packages of care

To initiate, follow up and advise patients of test results in partnership with clinical colleagues

To assess effectiveness of care through peer review, benchmarking and formal evaluation

To refer patients independently to other clinical disciplines as appropriate

To promote good health through screening and prevention interventions

To work collaboratively within the primary healthcare team to assist in the management of individuals with long term conditions

To assist with any recall or follow-up systems required to encourage patient attendance

To ensure that accurate, contemporaneous and complete notes of all consultations are made on the clinical system

Assess the patients ability to provide informed consent in keeping with the Mental Capacity Act and the Deprivation of Liberty standard

To utilise the audit cycle to evaluate the effectiveness and quality of interventions, implementing identified improvements as necessary

To understand and apply policies that support the identification of vulnerable children and adults, being aware of statutory requirements and local guidelines

Organisational:

To manage and prioritise time effectively

To assist in the operational management of the Nursing Department

To support the Nurse Team Leader in the planning and development of nursing services and in the delivery of Practice objectives.

To work with the Practice Manager in supporting IPC at the Practice, completing regular IPC audits and presenting the report to the Practice Manager for discussion and action at the monthly Education and Training Meetings.

To assist with the supervision of student nurses on placement as required

To assist with the training and mentoring of medical and nursing students when required

To assist in managing junior nursing staff including Healthcare Assistants, providing mentoring and supervision where appropriate

To participate in the induction of new staff and students where required

To liaise effectively with all members of the internal team and external agencies

To participate in Team/Clinical/Practice meetings as required

To work with the nursing and administrative teams to develop robust working protocols to maximise the use of available resources

To participate in the review of patient complaints and clinical incidents and identify learning utilising a structured framework

To attend all mandatory training

To ensure continuation of own personal and professional development
