Job summary

We are seeking an enthusiastic Advanced Nurse Practitioner to join our friendly practice. The successful candidate will provide high quality primary care nursing services to our practice population. You will work as an autonomous practitioner, working with practice policies, protocols and demonstrate safe clinical decision making.

The Surgery is migrating to a common platform for appraisals and performance monitoring for all GPs, Health Care Team and Reception and Admin team incorporating bench mark and standards set by the professional bodies (GMC/NMC) and regulators CQC.

Main duties of the job

To provide patient care from initial history taking, clinical assessment and diagnosis and treatment. To assess acute illness or chronic disease management. To have knowledge of QOF and the work required around this. Previous experience within a primary care setting is essential.

The successful applicant will be expected to lead on infection control and give support to the Health Care Team by holding regular team meetings along with performing appraisals and reviews.

About us

Toddington Medical Centre is situated in the heart of Toddington, Bedfordshire and serves all neighbouring villages and towns with a list size of over 7600 patients.

We are a teaching practice to the University of Cambridge Medical School hosting 4th year Medical Students along with supporting a number of schools with work placements.

Toddington Medical Centre is a Partnership of four general practitioners (One male GP and three female GP's) supported by a team consisting of Practice Nurses, Health Care Assistant and a Pharmacy Technician. We have additional role staff which consist of Social Prescribers, Care Coordinator and a Pharmacist.

The Partnership is contracted to the NHS Primary Care General Medical Services and some services are delivered co-jointly with the Primary Care Network (PCN).

Toddington Medical Centre is migrating to a high performing practice with an agenda for improving efficiency within the surgery and in the services that we deliver.

This means empowering our staff (both clinical and non-clinical) to develop and share our values, through adopting strong work ethics, good time management and staff to get trained/upskilled to ensure that our patient receive high quality care in a timely manner.

The current General Medical Service -Practice Model and NHS, is presently facing new challenges and we are committed to evolve and adopt best practices and develop new partnerships to serve our community.
