Job responsibilities

As a member of a multi-disciplinary clinical team, respond to and manage a range of patient health care needs within a primary care setting.

To provide assessment, diagnosis and treatment at first point of contact by attending to patients in a variety of non-clinical settings according to the patients needs.

To support a model of on the day care within practices.

To mentor and support staff in developing and maintaining clinical skills, where required.

To work across local care interfaces in order to deliver an integrated service intended to reduce waiting times for patients accessing urgent care, reduce unnecessary A & E attendances and reduce the demand for hospital admissions by accessing the right care at the point of need.

To work as an autonomous practitioner, providing clinical care (including telephone triage, diagnosis, treatment, referral, review or discharge) as appropriate, utilising a range of advanced skills and expert knowledge for patients presenting with undifferentiated and undiagnosed conditions.

To work as part of the practice team providing care in a variety of clinical and non-clinical settings (including residential care, patients homes and other settings as necessary).

To use broad clinical guidelines (NICE etc.) relating them to individual cases, providing assistance and feedback to the organisation and others on how these should be interpreted.

To signpost appropriately to other services in the identified healthcare pathway.

To prescribe as an independent non-medical prescriber and/or administer medications using Patient Group Directives (PGDs), according to agreed protocols.

To maintain accurate and contemporaneous records of clinical care provided, including adverse events using Systm1.

To provide ward round style consultations to the care homes associated with the PCN.

To provide clinical advice to other members of the team where appropriate including attendance at MDT meetings, planning meetings and case conferences as required.

Miscellaneous

Essential

• Vaccine and immunity status in accordance with UK Department of Health Guidelines
• Able to deal with sensitive issues e.g. sudden death/bereavement, child abuse, domestic violence
• Able to undertake home visits
• Hold a current driving licence
• Enhanced DBS disclosure
• Proof of eligibility to work in the UK
• Willing and able to work flexibly: The post requires working within core practice hours, but there may be the need to provide occasional sessions in the evening or at weekends

Qualifications

Essential

• Registered paramedic
• Evidence of continued professional development

Desirable

• Independent prescribing qualification
• Minimum level 3 (FHEQ level 5/6) accredited emergency care practitioner /autonomous practitioner or equivalent
• Attended X-ray/IRMER course

Experience

Essential

• Ability to manage and prioritise workload
• Proven ability to work autonomously and as part of a team
• Previous experience of non-medical prescribing and using PGDs
• Experience as an autonomous practitioner within urgent and / or primary care
• Knowledge and understanding of adult and child safeguarding issues
• Understanding of clinical governance and its significance and impact upon practice
• Good communication skills
• Competent IT user including clinical systems and email
• Record keeping skills; maintaining accurate, contemporaneous records appropriate to the consultation
• Able to work autonomously, using own clinical judgment to form a diagnosis
• Skills for managing minor injuries
• Able to give relevant discharge advice
• Experience of following referral pathways
• Able to work cooperatively with the multi-disciplinary team
• Able and willing to develop professionally

Desirable

• Experience of the working with care homes
• Understanding of the H.P.C Code of Professional Conduct
• Experience in using I.T based patient documentation system (TPP System 1 and EMIS Web)
• Experience of triage/initial assessment
• Able to record/ interpret ECG
