Job summary

To work as part of The House Partnership management team, in a developmental capacity. To identify and develop the practices strategic plan, building a robust digital infrastructure whilst maintaining governance compliance and the optimum patient experience.

Main duties of the job

Lead on developing systems and strategy to support services provided by The House Partnership.

Improving patient experience and general health and wellbeing.

Supporting improved access for patients on a range of integrated projects to enable staff to work together more effectively and sustain the future of general practice. To assist and progress specific network services or projects, and support network staff interacting with the practice on a day to day basis.

To ensure quality and improvement (CQC) integrating the ways the practices work together to improve efficiency and reduces duplication.

Extracting analytical data to produce data packs and present so as to support decision making and planning of the practice management team. Areas of scope include demand, capacity, utilisation data, staff, and patient experience.

To take consideration the population's health needs and health inequalities within our community.

To support delivering the practices Directed Enhanced Service framework (DES), Impact and Investment fund (IIF) requirements, and meet the strategic aims set out by the practice.

About us

Greystone House Surgery is a modern friendly Practice in Redhill representing one of the three sites which make up The House Partnership between Greystone House Surgery, Caterham Valley Medical Practice and The Moat House Surgery.

Training for this role will take place at Greystone House Surgery, 99 Station Road, Redhill, RH1 1EB. Cross-site working between the Merstham and Redhill sites may be required however this role is mostly based at Greystone House.

Job responsibilities

To lead on communications with patient facing services, to include branding, patient experience, practice identity, website, cloud telephony, health campaigns, social media, use of the NHS App.

To liaise with Patient Liaison Officers leading on responding to patient feedback, social media, complaints and significant events.

To recommend and influence decision making in patient access, service and experience based on interaction with patients and the local community.

To lead with practice administrative and clinical staff in the achievement of IIF, QOF, CQC, DES and other contractual compliance.

To work closely with the Practice Managers in developing new technology to improve overall efficiency.

To work with staff to coordinate work with partners to enable the practice to deliver high quality services for their populations such as health and social care partners, statutory agencies, voluntary sector, neighbourhood groups etc.

Work with clinicians to analyse data to identify population health needs. Design and deliver projects aimed at improving these outcomes.

Work with administrative teams with a view to population health and proactive disease management systems.

Submit regular high-level project reports showing progress against objectives and recommend developmental options.

Work with managers to create and maintain accurate documentation in relation to standard operating procedures, contacts, and decision-making.

Work with practices to achieve nationally set targets and support population health management.

Identify opportunities for projects that enable the practice to deliver its strategic aspirations and vision. Assist with project delivery and reporting.

To support improving Capacity and Access, to include Enhanced Access and digital triage.

Interrogate and analyse data to assess demand, capacity, and activity to support the delivery of project reporting and project outcomes, to include Enhanced Access.

Support communication within the practices and network projects where they overlap with practice delivery.

Build relationships and facilitate cooperation between the practices and at neighbourhood level to improve patient care delivery.
