Job summary
The Stokes Primary Care Network comprises of 2 practices - Bradley Stoke and Stoke Gifford Medical Centre. We are based in South Gloucester but within close proximity to North Bristol.
Being part of the Clinical Pharmacy team in General Practice is part of an exciting programme of transformation to develop a new model of care which addresses our ambition to deliver a person-centred, co-ordinated care. Clinical Pharmacy in the General Practice model is supported by the direction of national policy including the Five Year Forward View and GP Forward View where there is a need to better utilise the role of pharmacy within primary care to pro-actively help patients stay safe and well and out of hospital as well as helping to reduce the demands on general practice. This role is part of the PCN ARRS scheme which is part of this new way of thinking.
Main duties of the job
The pharmacy technician will support the lead pharmacist to deliver the Clinical Pharmacy in General Practice model of care, including supporting and assisting a cluster of GP practices. The purpose of this role is to lead improvements to maximise safe, cost effective best practice in prescribing to improve the quality of patient care. The post holder will help patients to get the best from their medicines through effective medicines reconciliation after discharge and on other transfers of care, medication use reviews, identification of patients suitable for electronic repeat dispensing, promotion of online ordering, providing assurance of monitoring of high risk drugs, and handling of prescription queries within the areas of competence. The role will also include training and development of practice based prescribing clerks and administrators to support improved co-ordination and effective pharmacy administration within general practice, especially with relation to repeat prescribing systems and processes.
In addition, the post holder will be responsible for encouraging the development of better understanding of the principles of medicines optimisation throughout the practice teams and promoting good practice in with therapeutic developments.

Bradley Stoke Surgery and Stoke Gifford Medical Centre are in the process of merging partnerships (with an expected merge date in April 25). The combined 40,000 patient population will be supported by a team of 130 clinical and administrative staff and delivered from 3 purpose built medical premises. The new Partnership will act as a single Primary Care Network with a single list and teams that will work together to provide excellent patient care.
