Job summary

If you are enthusiastic and highly motivated we would like you to join our recently formed The Stokes Primary Care Network (PCN) team. The PCN consists of 4 Practices: Bradley Stoke Surgery; Concord Medical Centre; Coniston Family Practice; and Stoke Gifford Medical Centre.

Main duties of the job

The successful candidate would provide managerial support to the PCN Board and would work closely with the PCNs Clinical Director and PCN Board Members, consisting of GP Partners and practice managers. The incumbent would be supported primarily by the practice managers and would undertake the role of managing PCN Board-level meetings to ensure that the PCN Board is meeting its contractual obligations from an organisational perspective. The post holder would be expected to work closely with all PCN Board Members and to be an integral part of the Board; a more detailed job description and person specification are attached.

Job Summary and Primary Responsibilities

The Stokes Primary Care Network (PCN) is a closely aligned network of four General Practices in South Gloucestershire, nearby to Southmead Hospital and serving a population of 60 000 residents.The Practices are Bradley Stoke Surgery, Concord Medical Centre, Coniston Medical Practice, and Stoke Gifford Medical Centre (Dr Peter Young, Clinical Director). In 2019, as part of general practice reform,the PCN was formed to support the implementation of the NHS long-term plan and to deliver new contract services.

The PCN Development Manager will support the Clinical Director and PCN Board at network level to develop and implementservice improvement and transformation work aligned to local and national priorities. This includesbuilding on current primary care provision and working proactively with the wider network to ensure we can offer our local population sustainable, proactive, personalised, coordinated and more integrated health and care. The role entailsbuilding effective working relationships across the network area, working closely with the clinical director,engaging stakeholders across different organisations and disciplines, coordinating work-streams, projects, tasks and events, and collating and monitoring risks and issues. The work supports thevision of Primary Care Networks to deliver care in a more coordinated way so that we treat and support people where possible within their local communities.

The post holder will be expected to have a critical role in providing strategic development and project management support to PCN Clinical Directors and management leads to implement thelocal vision of integrated provision and place-based care. This will involve the design and delivery of specific changes to the provision of care across primary and community services in partnership with local health and care providers, covering engagements at both Clinical Commissioning Group (CCG) and PCN Levels; and ensuring thespread of good practice and learning from different PCNs.

This role is central to the local transformation programme of work and as such is subject to change and development over the next 12 months in line with the emerging needs of the service and system locally.
