Job summary
Ascent PCN Ltd is looking to
recruit a Digital and Transformation Lead to work alongside our Clinical
Director, Business Manager and Practice Managers to provide support to the PCN.
This is an exciting opportunity for a motivated individual who can work
independently and is flexible in responding to the developing nature of the
role.
The main base will be Asplands
Medical Centre and Woburn Surgery but with flexibility to work and travel
across all GP practices with the PCN Network.
Working hours: up to 22.5 hours
(3 days) per week.
Main duties of the job
The core objective of the PCN
Digital and Transformation Lead role is to support the PCN, its additional
roles and the constituent practices to deliver ongoing improvement to services.
The role will require experience in service and quality improvements with good
knowledge of organisational change, ideally within a primary care setting.
To work in partnership with the
PCN leadership and management team, identifying and developing relationships
with PCN additional roles, practice staff, private providers, the local primary
care team, neighbouring PCNs and other key partners to enable good working
relationships and promote sharing of best practice.

- Evidence of Advanced Education
- Level 8 or 9 in English and Maths or equivalent.
Experience

- Experience of working in general practice or similar environment with a good working knowledge of general practice systems - including IT, processes and procedures.
- Experience of leading people and change projects.
- Experience of interpreting national policy and implementing this through programmes of work.
- Experience and understanding of evaluating and measuring the performance of services.
- Understanding of the Data Protection Act and UK GDPR
Desirable
- Strategic understanding and experience of working in the NHS.
- Experience of developing primary care provision at scale, including working with a federation or PCN.
- An appreciation of the new NHS landscape including the relationship between individual organisations, PCNs and the commissioners.
- Experience of using GP IT systems .
